Considering the healthcare issues affecting Jordan town and its surroundings, there is a need for critical stakeholders to foster corporate social responsibility (CSR). For the change proposal, some stakeholders will participate directly in decision-making while others will engage
through policy formulation. The change proposal will encompass the establishment of an enthusiastic community health personnel to coordinate precise outreach, targeted check-up initiatives, and home visits to educate the residents about quality care service provisions. There
must be an appropriate assessment of the healthcare system in the rural region (Adler, 2012). Most people do not have access to quality healthcare services, cannot afford education and care services, and lack access to high-quality and safe basic facilities such as food and other essential services (Winterton et al., 2014). Hence, the central role of the project staff would be identifying healthcare concerns among the vulnerable groups and adopting the appropriate measures to address them, such as drafting policies to improve social and economic statuses in the region.

Based on the organizational change, community health components will only work in collaboration and correlation. Potential Barriers to Change
In implementing community and organizational change, there is a likelihood of resistance to the proposed change. Invention and creativity would be the initial barriers to change in Jordan city. The area lacks essential facilities and technology that might facilitate the appropriate
implementation of the project. The high poverty rate in the region has deterred technological advances, and people cannot afford installing essential equipment for appropriate education facilities (Capella University, 2021). While the government might provide the resources through
the city council, modern technology for effective implementation is lacking. The field staff will face challenges since access to healthcare services and products such as pharmaceuticals is limited. Other potential hindrances might emanate from social drawbacks because the society seems conservative on its beliefs. Indeed, the community seems not to embrace diversity. As a result, the people have not been supporting appropriate changes and growth initiatives (Capella University, 2021). Some political hitches might occur within the city council staff in financial resource allocation leading to halting the entire change proposal (Adler, 2012). To successfully overcome any possible barriers, all stakeholders must coordinate and collaborate in all change steps. Varies social entities such as schools and other public facilities will be utilized in sensitizing the community.
Stakeholder Communications
Proper and successful implementation of the organizational change must incorporate all stakeholders (Silvius, & Schipper, 2014). The critical stakeholders involved in the efficient implementation of the community change include Jordan city council officials, Vila Health, the business people within and around the city, the entire population, learning institutions, and other social service and care centers (Stroh, 2015). Religious groups must also be incorporated to give their views concerning the community change. Proper communication channels must be set for all
stakeholders to warranty consistency and efficacy in all steps involved. Collaboration and effective communication among stakeholders are imperative to eradicate potential friction and conflict of interest (Adler, 2012). There will be a need for experts to gather relevant statistics and conduct structured interviews and case studies encompassing community members for decision-making processes.

Need Statement
The proposed community change will address the limited access to social services, including healthcare and schools. The change will assist in detecting chronic disorders, including diabetes, and educating the community on disease awareness to improve their social well-being. It will first deal with around 250 community members with psychological complications. According to a Jordan middle school teacher, a middle school kid committed suicide before graduation. There is a need to have a facility for discussing mental health issues (Capella University, 2021). Some barriers might include financial constraints and community resistance. Different interviews and social briefings enabled the collection of essential information regarding the need for change. Some NGOs, religious groups, and the city municipality have set initiatives to address the social issue.
Program Description
This is typically community healthcare and social outreach, which focuses on civil education, disease screening, and lifestyle improvement. The initiative will be implemented by competent public and community health officials, including physicians and nurses in specific regions such as learning institutions and social gatherings. There will also be counseling for the drug addicts and rehabilitation services.
Goals and Objectives

The funding organization’s goals are preventing poor care services, minimizing medical costs through preventive care, which aligns with the change initiative.
