What are the appropriate pharmacological therapies to be prescribed for Johnathan?

          According to Jonathan’s mother, 2 to 3 days before the worsening cough and wheezing, Jonathan had a viral upper respiratory infection with a runny nose and low-grade fever of 101.0 degrees F orally, with loose cough – that could be initial signs of acute upper respiratory infection [URI] (Beta healthy, 2019). One of the comorbid conditions or risk factors of asthma exacerbation among children is upper respiratory infection (Hollier, A., 2018). A study looking at the role of viral respiratory infections in asthma and asthma exacerbations reports that with existing asthma, viral respiratory tract infections can have a profound effect on the expression of disease (Busse, W., Lemanske, R., Gern, J., 2010).  The authors go further and states that viral respiratory tract infections, most frequently with rhinovirus, are the predominant microorganisms associated with asthma exacerbations. Jonathan seems to have an asthma exacerbation induced by a URI. The appropriate pharmacological therapy for Jonathan will be a short acting bronchodilator such as albuterol, that stimulates beta 2 receptors in the lungs. (Hollier, A.). Jonathan will be prescribed albuterol 90mcg, 2 puffs q 4-6 hours that he will be using as rescue inhaler. Jonathan’s asthma is identified as mild intermittent asthma. According to Hollier, A.  with mild intermittent asthma, a short acting bronchodilator is the treatment of choice for exacerbations (p.664). Furthermore, according to Tibble, H., Tsanas, A., Horne, E., Horne, R., Mizani, M., Simpson, C. Sheikh, A. (2019), asthma therapy typically follows a fairly linear path, beginning with a short-acting bronchodilator in the individuals without persistent asthma symptoms and adding preventative treatments and long-acting bronchodilators in the individuals with more persistent asthma symptoms.

What information is necessary to provide to Johnathan and his mother regarding asthma exacerbation?

Jonathan and his mother will be provided information about how to identify and minimize known asthma triggers by avoiding allergens and irritants. Respiratory irritants can be tobacco smoke, wood smoke, perfumes, pollution dust, etc. (Hollier, A., 2018). Jonathan will be instructed to take his medications as prescribed, learn early signs and symptoms of exacerbation such as severe shortness of breath, chest tightness or pain, and coughing or wheezing, low peak expiratory flow (PEF) readings, if a peak flow meter is used and also symptoms that fail to respond to use of a quick-acting inhaler. The most important information that will be provided to Jonathan and his mother is to implement an asthma action plan, a preplanned medication plan for asthma exacerbations. The correct way to use the inhaler, spacer and other medications will be reviewed with Jonathan and his mother. Mother will also be encouraged to give the influenza vaccine to Jonathan every year to decrease his change to catch the flu that can exacerbates asthma attacks.  

What is an appropriate clinical assessment tool to be use with Johnathan?

          In my opinion, the best clinical assessment too to be used with Jonathan is the peak expiratory flow (PEF). Keeping tract of the PEF values is on way to know if the symptoms of asthma are in control or worsening. During an asthma attack, the smooth muscles that surrounded the airways tighten ad cause the airways to narrow. According to WebMD (2019), the PEF meter alerts the patient to the tightening of the airways often hours or even days before the onset of the asthma symptoms. By using the PEF with the asthma action plan, Jonathan will know when to take is rescue asthma inhaler. 

What are the classifications of asthma?

The classification of asthma severity is as follows:   

Mild intermittent. Symptoms occur less than 2 days a week or less that 2 night per months and do not interfere with normal activities and lung function test is 80% or more of the expected value. Exacerbation is brief.

Mild persistent. Symptoms occur more than 2 times a week, but less than one time per day and 3 to 4 nights per month. 

Moderate persistent. In moderate persistent asthma, the symptoms occur daily with some limitation. Lung function test is abnormal with more than 60% and less that 80% of the expected value (Buttaro, T., Trybulski, J., Polgar-Bailey, P., Sandberg-Cook, J. 2017). 

Severe persistent.  There is continual symptoms or frequent nighttime symptoms more than one night per month with severely limited activities.

How would you as the NP address his mother’s concern regarding providing an inhaler at school?

          I will suggest Jonathan’s mother to schedule a conference with teachers and other school officials to go over the details of Jonathan’s  and the plan and any other details they should know including need of having his inhaler with him, the correct use of the inhaler, location of the inhaler, and signs of trouble breathing to ensure that the school nurse, the principal and his teacher has a copy of his asthma action plan and to bring his inhaler with him all the time. The school should also know when to call Jonathan’s doctor and when to call 911. The mother should ensure that the action plan has the doctor’s phone number, their preferred hospital (emergency room), as well as contact numbers for her, other guardians if applicable, and a trusted friend.

What is an appropriate plan of care for Johnathan?

An appropriate plan of care will be to use a PEF to monitor his respiratory status and control signs and symptoms of asthma. Jonathan’s should have his rescue medication with him all the time. Jonathan will need to avoid asthma triggers such as irritants. Follow up with Healthcare provider is also imperative to monitor evolution of his asthma.

Johnathan, age 7, presents to the office with symptoms of worsening cough and wheezing for the past 24 hours. He is accompanied by his mother, who is a good historian. She reports that her son started having symptoms of a viral upper respiratory infection 2 to 3 days ago, beginning with a runny nose, low-grade fever of 101.0 degrees F orally, and loose cough. Wheezing started on the day before the visit, so 

Johnathan’s mother started administering albuterol metered-dose inhaler (MDI) two puffs before bed and then two puffs at around 2 AM. The cough and wheezing appear worse today, according to the mother. He had difficulty taking deep-enough breaths to inhale this morning’s dose of albuterol, even using the spacer.

Johnathan has been a patient at the clinic since birth and is up to date on his immunizations. His growth and development have been normal, and he is generally healthy except for mild intermittent asthma. This is his first asthma exacerbation of the school year, and his mother expresses a concern about sending him to school with an inhaler.

Johnathan is afebrile with a respiratory rate of 36 and a tight cough every 1 or 2 minutes. He weighs 45 pounds (20.5 kgs.). The examination is all within normal limits except for his breath sounds. He has diffused expiratory wheezes and mild retractions. Pulse oximetry readings have been 93% of oxygen saturation.

It is essential to teach patients and family how to monitor signs and symptoms, and take appropriate action of asthma exacerbations. The recognition of early symptoms and decreased lung function may require medications adjustments (Woo & Robinson, 2016, p. 929).

What is an appropriate clinical assessment tool to be use with Johnathan?

 Monitoring patients with asthma is a continuous process, beginning with the initial diagnosis. The Expert Panel Report 3: Guidelines (NAEPP, 2007) recommends ongoing monitoring of the following six areas: signs and symptoms, pulmonary function, quality of life and functional status, history of asthma exacerbations, pharmacotherapy, and patient–provider communication and patient satisfaction (Woo & Robinson, 2016, p. 933).

Correct answer:   This is an advertisement.  You can tell because of the term “sponsored content.”  According to dictionary.com, sponsored content can be defined as: “material in an online publication which resembles the publication’s editorial content but is paid for by an advertiser and intended to promote the advertiser’s product.”

Please note that an advertisement can look 100% like an article.  It can be long, can contain information, can be written by an author, can have a title, etc.  It is an advertisement simply because it is designed to sell something.  Here are some other identifying features:  Typically, individuals or organizations that are trying to sell their product, purchase the space (whether visual, textual, aural or otherwise) from the magazine or program where the ad appears.  Additionally, these adds are often created by advertising departments or organizations but can be made to look like they are created by everyday individuals. 

4. Examine the picture above. Does this post provide strong evidence about the conditions near the Fukushima Daiichi Power Plant? Explain your reasoning.

Correct answer:   No. Nowhere does it say that this picture is taken near Fukishima. We also don’t know when it was taken. We don’t know anything about the person sharing the photo.

Also, while on one hand it might be true that radiated flowers look like this as the caption claims, the source is not claiming that these flowers are examples of what happened in Fukishima. This is a way for a source to be honest and misleading at the same time.  This is a common online trick that often works on those that haven’t learned to look for it.

On the other hand, it’s possible that these strange daisies are a naturally occurring variety of daisies — maybe these daisies look like this everywhere they grow on earth! The picture is asking you to make an assumption that something is wrong with the daisies.   

 Many of you missed this one but will be savvier in the future :). 

5. Why might this tweet be a useful source for your research? Why might it not be useful? Explain your reasoning.

It’s an okay source of data. It’s from a nationally recognized polling organization, and the tweet has a link to the study, which you can follow for more information. Just because it’s a tweet doesn’t mean it’s automatically a bad source for your research. Important scientists, historians, and journalists use social media and often share information on these platforms with links to more information. 

Some of you only answered one part of this question.  Of course, there are several ways to assess the credibility of the tweet, and many of you did this effectively.  However, this question was not about credibility it was about usefulness.  When determining whether you want to use a source, credibility is only one factor that you need to consider:   

The truth is that absolutely any source could be both “useful” for some research purposes, and “not useful” for others. It all depends on what the research questions are.  A tweet might be a bad source if the researcher wants to prove or understand the nuances or specifics of a situation.  A tweet can hide biases, and/or take numbers out of context.  Another problem – many audiences might not trust evidence that comes from a tweet.

On the other hand, this tweet could be an excellent source for a researcher investigating how gun control attitudes are expressed through popular media.  In those cases, the credibility of the source might not even be relevant.  The key is for researchers to make good decisions about whether a source is sufficiently credible, relevant and appropriate to serve their research purposes.  

1. The “We Know You’ve Got a Story” banner is an advertisement. (a) The banner appears to have been put there by the editor of the online news publication to influence the thinking of the readers and persuade them to purchase it. Advertisements are used to grab the attention of consumers and attract them to buy a product or service (Alalwan, 2018). Consumers’ purchase decisions heavily rely on advertisements as they give the consumers reasons for purchasing a product or service. The banner can be effective in influencing consumers’ purchase intentions by invoking their thoughts on the stories in the news publication. 

           2. The “Should California Stop Growing Almonds” block is an article that provides information on the growing of almonds in California. (a) Although the author of the article is stated, it does not qualify to be an advertisement as the sponsor is not indicated. Advertisements in news publications are mostly identified with labels that make the ads different from other articles (Amazeen & Muddiman, 2018). These labels are designed to make advertisements look different from other news articles, although it can still be challenging to determine what comprises an advert due to the tactics publishers use to present advertisements as articles

           3. The “Real Reasons Women Don’t Go Into Tech” block is an advertisement, which informs readers about women’s involvement in careers in technology. (a) What makes it easy to identify the block as an advertisement is the inclusion of the words “Sponsored content” which is written in a different font and color to differentiate it from the other information (Amazeen & Muddiman, 2018). The word “sponsored” is commonly used to identify ads that are cloaked in the guise of stories. The advertisement targets a specific audience (women) or even spreads awareness because the issue is of public interest, which can be described as covert & public-serving advertising (Wojdynski & Evans, 2020). 

           4. The post provides strong evidence of how the Fukushima Daiichi Power Plant has affected the surrounding environment. The article is accompanied by a picture that portrays the impact of the power plant on the vegetation in the surroundings. The reader can easily relate to the author by observing the abnormalities in the flowers. The deformations suggest the radiation from the nuclear power plant disaster affects the structural development of the plant’s flowers (Matsala et al., 2022). The post can be used to show what happens when plants are exposed to nuclear radiation. The picture used in the post communicates to the reader by showing the defects associated with the nuclear accident in the environment.

           5. The tweet might be a useful source of information for research because Moveon.org is a reputable organization. The information in the tweet was obtained from survey data that was collected from 816 gun owners, which is not a small sample size. The size of the sample influences the validity of the information collected from the sample. (a) The tweet might not be useful because there could be factors that influenced the decisions made by the individuals interviewed. Nurses can ensure lawmakers hear their voices in several ways. Through lobbying, nurses can help legislators make informed decisions regarding nurse practice and the public’s interest. The social worker had limited assessment information and went with what her gut told her instead of getting the necessary detailed assessment and the use of diagnostic tools necessary to come to a diagnostic conclusion of schizophrenia. The social worker professional obligation is to not only to make a clinical diagnosis but in doing so looking at the client’s general medical health, psychosocial and environmental concerns, and to assess the client’s level of functioning with the presenting problem (Cooper, 2018). I believe this was poor judgment on the social worker’s behalf. Explain the potential impact of this diagnosis immediately and overtime if the “tentative” diagnosis is a misdiagnosis. The impact of giving a tentative misdiagnoses can not only have immediate effects but also have long term effects. Schizophrenia is a type of psychotic disorder however other things can cause psychosis such as drugs and medical conditions. Schizophrenia is also treated with pharmacological medications such as anti-psychotics that can have serious side effects. A diagnosis in the client’s medical records can have profound effects on all aspects of life. The military discharges individuals for mental health diagnosis, and with this diagnosis, she would lose all military retirement benefits as well. Giving the wrong diagnosis is irresponsible and unethical.

When may it be appropriate to use a provisional diagnosis

A provisional diagnosis is given when the social worker does not have enough information to make a diagnosis. The specifier provisional is only used when the social worker feels strongly that the full criteria of the diagnosis will be met once more information is obtained (American Psychiatric Association, 2013). Such as when a diagnosis has a specifier of a specific time length requirement. When would diagnosis as other specified and unspecified disorders? The social worker can use the other specified disorder when the presenting problem’s presentation does not meet the criteria for any one specific category within the diagnostic class and must state specific reasons (American Psychiatric Association, 2013). The unspecified disorder is used when the social worker chooses to not specify the specific reasons the criteria are not met (American Psychiatric Association, 2013). According to the DSM-5 (2013), the difference is entirely clinical judgment and left up to the social worker.

As we are aware, traumatic experiences can have a long-lasting affect on a person’s mental state especially if the person does not seek treatment. In the case study, Ms. Evans was serving in the military on tour in Iraq and she began to experience, and express specific issues that caused concern for her mental state. Some of the “red flags” identified in the case study that could be viewed as a possible symptoms of a mental health disorder include: thinking and expressing her fear of the world ending in 2020 due to the voices in her head, self-diagnosis of depression and appearing fearful by those close to her. According to Roberts and Trockel (2015), there were other concerns, such as changes in her behavior, for example there was a decline in her job performance and her ADLs or self-care have decreased. It was noted that Ms. Evans was positive for THC (tetrahydrocannabinol), which may be problematic due to certain symptoms displayed could be enhanced by the substances she is consuming. One must consider several things when dealing with substance use, which include frequency and self-medication likelihood. These should be considered when there is trauma involved along with the severity of symptoms being exhibited. The more information gathered during the assessment portion, the higher probability of having an accurate and effective treatment plan (Neukrug & Fawcett, 2015). A misdiagnosis due to lack of information can be detrimental to a client, especially when medications are involved, because it can cause adverse effects with the client in turn possibly worsening the symptoms or situation. According to Paris (2015), “When a tentative diagnosis is a misdiagnosis and that diagnosis is then communicated with the patient it can interfere with treatment and pigeonhole them into categories that they do not belong in, resulting in self-stigma.”

In my professional opinion it was premature to share the tentative diagnosis with the client based off the information gathered, because there are several diagnoses that have similar symptoms. I believe further assessment needs to be completed, with the assistance of a psychologist and/or psychiatrist. Provisional diagnosis may be appropriate to use when the clinician is certain the full criteria of a diagnosis will be met once enough information is obtained (American Psychiatry Association, 2013).Clinicians can used other specified with a diagnosis if a client does not meet the full criteria and documenting the criteria(s) not met. Clinicians may use unspecified in the same manner with a diagnosis; however, they are not required to document the criteria not met. According to American Psychiatry Association (2013), there is no requirement placed on the clinician on which of the two they can use.
