DNP graduates are expected to apply research findings and integrate nursing science into evidence-based practice. To develop your ability to engage in this high level of nursing practice, you will analyze the strengths and weaknesses of a research study over the next several weeks using the concepts presented throughout Weeks 1 through 9 of the course. Your final analysis will be a 5- to 7-page paper in APA format, that includes the following:
1) Write an introduction ending with a purpose statement (e.g. “the purpose of this study is…).
2) Provide a two paragraph overviews of the study.
3)  Identify three strengths of the study and support your selection (i.e. why is this a
strength)
4)  Identify three weaknesses of the study and support your selection (i.e. why is this a
weakness)
Note: The strengths and weaknesses should be in relation to:
· Design
· Sampling
· Data collection
· Statistical analysis
· Results and discussion.
5)  Propose changes to improve the quality of the study capitalizing on the strengths and
6) improving on the weaknesses you identified in the study.
7)  Summarize the implications for nursing practice.
8) Write a conclusion for your paper
The purpose of the analysis is to help you develop a deeper understanding of the research process, to inspire you to think critically and deeply about research on a specific topic, and to strengthen your ability to integrate research findings into evidence-based nursing practice. This assignment also gives you practice in analyzing the research literature, which will support you when you begin your DNP project.
Due Wednesday October 25, 2017 before midnight!
Writing (5 points): Up to 5 points (20%) may be deducted for grammar and APA style errors.
To prepare for this week’s assignment complete the following:

  • Select      one of the four research articles cited in this week’s required readings (SEE ATTACHED PDF FILE).
  • Review      the various quantitative research designs presented in the textbook      readings and research articles and discussed in the “Musings: Aligning      Research Question and Methodology” media.
  • Consider      the research design used in your selected article. Ask yourself the      following questions. Is the design appropriate for the study? Would a      different design provide better results?

Chapter 10, “Quantitative Methodology: Noninterventional Designs and Methods
Chapter 10 describes the principles of research design and those foundational concepts that influence the selection of an appropriate quantitative design: causality, bias, manipulation, control, and validity.
Chapter 11, “Quantitative Methodology: Interventional Designs and Methods”
This chapter builds on the material presented in Chapter 10 and discusses how to select the most appropriate quantitative research design for addressing a particular research problem.

Of these, nearly 700,000 children were found to be victims of maltreatment: 18% were victims of physical abuse and 78% were victims of neglect (CDC, 2014). Child sexual abuse makes up roughly 10% of child maltreatment cases in the United States (CDC, 2014). The CDC considers sexual abuse at any age a form of violence. Child abuse of any kind can lead to an increased state of inflammatory markers in adulthood, as well as multiple physical illnesses and high-risk behavior such as alcoholism and drug abuse. If a PMHNP identifies child abuse, there may be a need to report the abuse to authorities. Once able to provide treatment, the PMHNP can be instrumental in reducing the long-term effects of child abuse.

While financial managers have no time machines or crystal balls, they do have expense forecasts. Expense forecasting is one of the preeminent tools that financial managers can use to prepare their organizations for future fiscal turbulence. In this Assignment, you will examine a scenario and generate a corresponding expense forecast in Excel.
Before pursuing an opportunity or making a major purchase, financial decision makers must first ascertain if the expenditures are justified. Determining whether a new process, system, or purchase will yield worthwhile returns is no easy task. However, managers have a variety of tools to help them decide whether the new expenditure is warranted. Analyzing a venture’s benefit/cost ratio, marginal profit and loss statement, and break-even points enable nurse managers to make educated decisions about how they choose to commit their funds.

Your department has performed 20,000 procedures during the first six months (January–June) of 20X1. Spending during that period of time was $210,000 for fixed expense items and $1,200,000 for variable expense items. Of those amounts, $50,000 of fixed expense money was spent on preparing for a Joint Commission survey. Volume is anticipated to be 10% higher in the second half of the year. On November 1st, two new procedure technicians will begin work. The salary and fringe benefit costs for each are $96,000/year. Based on the information provided, prepare an expense forecast for 20X1.

You are examining a proposal for a new business opportunity – a new procedure for which demand is expected to be 1,400 units the first year, growing by 600 units a year thereafter. The price charged per procedure is $1,000. The collection rate is anticipated to be 80%. Each procedure consumes $300 of supplies. Salary cost is estimated to cost $540,000 each year, fringe benefits are 25% of salaries, rent for the facility is $55,000/yr and operating cost are $120,000/yr.

Break-Even Analysis Scenario

You can charge $1,075 for a new service. Demand is anticipated to be 8,000 units a year. Your business is able to handle up to 16,500 units annually, so capacity should not be a problem. The average collection rate is 80%. The new service has annual fixed costs of $4,700,000. Variable cost per unit of service is $420.
Question: Use break-even analysis to determine if this new service is financially viable. If the business is not financially viable, what steps could you take to make a case to proceed with implementation?  Explain your decision.

Benefit/Cost Ratio Analysis Scenario

You are considering the acquisition of a new piece of equipment with a useful life of five years. This new technology will make your clinical operation more efficient and allow for a reduction of 10 FTEs. The equipment purchase price is $4,500,000 plus 10% installation fee. The purchase price includes service for the first year, an item that has an annual cost of $10,000. There is a potential for additional volume of 150,000 units in the first year, growing by 30,000 each year thereafter. The price charged per unit is $15.00 with a 50% collection rate. The staff being eliminated are paid $12.50 per hour. The fringe benefits rate is 20%. The hurdle rate is 7.5%.

In this Assignment, you will identify clinical areas of interest and inquiry and practice searching for research in support of maintaining or changing these practices. You will also analyze this research to compare research methodologies employed.

    Terminologies in nursing are essential in underscoring the influence of continued learning in promoting the concept of care. This is based on the understanding that knowledge keeps improving and healthcare professionals sometimes find themselves going through stacks of big data in order to build relationships. This has brought the concept of big data which has also influenced the nature and capacity of each healthcare professional or business to integrate big data to their programs (Laureate Education (Executive Producer), 2012). However, in the absence of a standardized nursing terminology, the process of processing data, accessing information, improving knowledge, and sharing the wisdom of this process would be long, wasteful, and detrimental to the growth of nursing. To this extent, it is noteworthy that standard nursing terminology is of great importance to nursing. To begin with, it helps in organizing, and applying knowledge through a process of informatics (Rutherford, 2008). Terminologies that have formed part of the long-term practice are made essential to the continued development of the system and therefore promotes common understanding of the issues that affect nursing care. They would utilize the data collected to support the knowledge they already have, or to improve on it, and by doing so, become more helpful to the society (Rutherford, 2008). This process has attendant benefits such as promoting effective communication between healthcare providers. Furthermore, it serves to increase visibility of nursing interventions and improvement in patient care in the long run. Enhanced data collection helps to evaluate nursing outcomes and by doing so, generate greater adherence to standards that promote care. This is because it creates an equalizing framework for different healthcare providers in the long run. Finally, the standardization of terminologies has the essential potential of facilitating the assessment of nursing competency (Rutherford, 2008; McGonigle & Mastrian, 2017).     Pain and sleep/ wake disorders have become prevalent in the recent years. Previous research done indicates that sleep/wake disorders are more common, underdiagnosed and mostly affects elderly people who suffer serious consequences (Miguel, & Albuquerque, 2011). Patients suffer from insomnia and prevalence of the disorders increases with the elderly in the population being affected most. Clinicians and therapists make common misconceptions by assuming that the condition is an expected phenomena that is associated with old age and therefore is normal (Miguel, & Albuquerque, 2011). Contrary to therapists and doctor’s misconceptions, research conducted indicates that the increased disorders are a result of increased psychosocial comorbidities in the population.
    Recent research and publications indicate that pain and wake related disorders should be regarded and treated as a multifactorial geriatric syndrome and therefore should be addressed with high attention. Many therapists and clinicians consider enough and good sleep to be essential towards achieving better health (Barbehenn, & Wolfe, 2010). For purposes of this assignment, the paper involves a case study about Caucasian man with hip pain. The paper involves decisions concerning medication to prescribe.
    From the symptoms outlined in the Caucasian man case study and recommended prescriptions of medications, it is evident that the patient is suffering from Complex regional pain disorder which is also referred to as reflex sympathetic dystrophy. The syndrome involves chronic pain that either affects legs or arms. The syndrome occurs after an individual goes through a surgery, heart attack, and stroke or after an injury. Complex regional pain disease causes aren’t clearly understood and is mostly common with the elderly. Doctors and therapists recommend early diagnosis and treatment of the condition to increase chances of completely eradicating it.
    Symptoms of Complex regional pain syndrome include; decreased ability to move affected body part, swelling, sensitivity to cold and touch, changes in hair and nail growth, tremors, muscle spasms and atrophy (Jänig, & Baron, 2003). However, symptoms may differ from one individual to the other where swelling, pain, hypersensitivity and changes in temperature being most common. Several therapies and medications are recommended and below discussion outline various medications for complex regional pain.
    Decision Point 1
    Decision point 1 involved selecting between two drugs which were; Savella 12.5 mg that were to be administered orally in day one. The drug was also supposed to be administered in day 2 where 12.5 mg was to be administered twice. 7.25 mg was to be administered two times a day and 50 mg thereafter. The second option in making the first decision involved Amitriptyline where 25 mg would be taken during bedtime and 300 mg of Neurontin two times a day.
    Psychiatric-mental health Nurse Practitioners (PMHNP) should recommend Savella in the Caucasian case study. From the results and side effects outlined in the case study, they are similar to those associated to Savella. The drug is used to treat psychiatric and depression disorders and is referred to as norepinephrine reuptake inhibitor with selective serotonin (Barbehenn, & Wolfe, 2010).
    Savella is administered orally without food. By taking the drugs without food, tolerability of the drug is enhanced. According to the case study, the patient is supposed to return after 4 weeks. According to the patient, the pain had significantly reduced which is an effect of savella drug that aids in relieving pain. Savella drug is known to cause various side effects such as; nausea, vomiting, constipation, increased sweating, headache and the occurrence of hot flashes (Barbehenn, & Wolfe, 2010). According to the case study, the client is reported to be suffering from bouts of swelling with no main cause together with nausea and suffers from insomnia too. Therefore, it is evident that the side effects that emerged were as a result of the savella drug.
    By making the decision that a psychiatric-mental health nurse practitioner should opt for Savella dosage, it was to achieve and capture the side effects caused by the drug as well as its healing capability. Administering the drug to the patient was to also act as proof that savella is used to heal and relieve chronic pain. According to research, savella should be taken two times a day just like in the Caucasian case study. After four weeks of administering the drug, dosage can be reduced based on the patient’s medical condition and how best he is responsive to treatment.
    The expectations in decision one were that the client would have homicide and suicidal thoughts as a side effect. Savella is known to cause suicidal thoughts to clients taking the drugs (Barbehenn, & Wolfe, 2010). However, in the Caucasian case study, the client reports to claim that he had been suffering from suicide and homicide thoughts. Instead, the patients report that the future looks good. The difference was attributed to the patient’s positive thoughts about life. For example, the client at some time claims that one of the doctors he had met before had suggested for a wheelchair for the patient which he did not admit and instead said that he was okay and did not require one. In addition, he is optimistic about life since he had been recently engaged and was looking up to for marriage. Therefore, the patient’s positive attitude towards life made the client not suffer from suicidal thoughts and thus the difference between the expectations and actual results presented after usage of the drug.
    Clinical Decision point 2
    The Psychiatric-mental health nurse practitioner had an option of continuing with current medication which was Savella but this time lower it. The second option was to discontinue savella and instead start to use lyrica by administering 50mg twice in a day. The third option for the nurse was to discontinue savella and administer 50 mg of Zoloft twice a day.
    In clinical decision point 2, the psychiatric-mental health nurse should discontinue the use of savella since the client reported that the situation had not been cured. He is reported to claim that he suffered pain with a range of 4 out of 10 and wanted that to reduce to 3. Therefore, the nurse had to try a different drug and monitor how well the patient would recover from the condition.
    For Clinical decision point 2, the psychiatric-mental health nurse practitioner should recommend the introduction of Zoloft. From the results of clinical decision one, the patient continued to suffer from lack of sleep. Zoloft is therefore recommended to improve on sleep and the mood. The drug has several other uses such as treating; panic attacks, obsessive-compulsive disorders, social phobia, post-traumatic stresses and severe form of premenstrual syndrome disorder (Miguel, & Albuquerque, 2011).
    According to the Caucasian case study, the patient reported to have no suicidal and homicide thoughts. Meanwhile, the pain increased from 4 to 3 out of 10 and that the condition did not improve, instead, it worsened. However, the client no longer experienced difficulties in sleeping. Moreover, his blood pressure was reported to have increased but did not suffer palpitations. The improvements to better sleep can be associated with the introduction of the new drug Zoloft. The drug may be administered with food or without. The best times to take the drugs should be in the morning or after the evening meal. The drug is associated with many side effects and therefore, a doctor may recommend that a patient start on a low dosage and later increase it to control the effects. Some of the common side effects include; nausea, dry mouth, diarrhea, stomach upset and drowsiness.
    The expectations of using the drug included reduced pain from 4 to 3 out of 10. Though there were improvements in sleep, it was affected by the severe pain. It was expected that the patient would get enough sleep. Heart rate and pressure were expected to remain at normal rates. Research conducted does not show any side effects that cause increased heartbeat and pressure. The drug is associated with boosting a patient’s mood and the reason he never suffers from homicide and suicidal thoughts.
    The difference between the expectations and the actual results presented is attributed to the mix up of drugs. The body did not adapt to the sudden changes introduced and therefore reason as to why pain increased. The positive moods caused by the drug are a reason as to why the client does not have suicidal thoughts. Moreover, the patient was evaluated after four weeks. The time was a long period since it was a new drug. The psychiatric mental health practitioners should have recommended the patient to return to the clinic after a week in order to urgently monitor his progress. For example, if the patient visited the clinic after a week, the nurse would have been in a position to detect increased pain to 7 out of 10 and thus change the dosage or medication.
    Clinical Decision 3
    In clinical decision 3, the nurse had the option of using Savella but increase the dosage from 12 mg to 25 mg in the morning and 50 mg during bedtime. The psychiatric nurse practitioner had also an option to discontinue savella and instead introduce 50 mg of tramadol. In addition, the nurse had an option of introducing a new drug calexa with 12.5 mg.
    The PMHNP nurse should prescribe Salville. The drug should at this stage be increased to a higher amount. From the results of clinical decision 1, the patient had recorded improvements in her condition, especially on pain. Therefore, the client ought to have continued with the medication to improve the condition further.
    The expectations after deciding to continue with the drug were that pain would reduce to probably a rate below 4 out of 10 like he experienced after decision one. Further expectations were that the patient would now have peaceful sleep since Zoloft had already cured insomnia. At end of stage 2, the patient reported to only lack sleep due to pain. Therefore, since pain would be reduced through the introduction of more dosage, the patient would have enough sleep. At this stage, the patient was still optimistic that he would heal and therefore pain would end and therefore did not have homicidal and suicidal thoughts
    The difference between the expectations and the results was the increased pain. The patient reported complex neuropathic pain syndrome which was feared not to respond to pain medication. By the second re-introduction of the drug, the patient could have suffered from resistance to the drugs which resulted to increased pain (Barbehenn, & Wolfe, 2010). Tramadol and celexa were ruled because they were not good and were feared to cause severe side effects such as serotonin toxicity.
    How Ethical Considerations Might Impact Treatment Plan
    Confidentiality
    According to APA ethics standards, psychiatrists and psychologists are mandated to maintain the confidentiality of their patients. However, confidentiality is sometimes breached for example in instances where insurance companies require details about patients in order to make hospital bill payments (Kerridge, & Stewart, 2009).
    Patient Abandonment
    Psychologists are not supposed to abandon patients. However, in a multidisciplinary setting, psychologists are faced by ethical issues where at time patients are abandoned. According to APA standards, psychologists should provide pre-termination counselling (Rezai, &Zonenshayn, 2002).
    Clinical Research
    In most cases, psychologists are required to conduct and participate in clinical research especially in a multidisciplinary setting. However, ethical issues may arise for example in choosing appropriate and adequate control groups.
    Psychology’s Interface with Medicine
    Patients are treated with multiple medications. However, ethical issues may arise when too many drugs are recommended to the patient and at times end up being ineffective. APA standards stipulate that psychologists should provide services that are within their boundaries (Rezai, &Zonenshayn, 2002).

Dear Class,
 
This week (Week 2), we are attaching tables and/or figures directly to the discussion forum.  Here are some tips in order to do so effectively.  I have provided some steps in order to assist with this problem.  This is necessary so that other students can effectively respond to you.

1. Save graph from word document as a picture on your computer (PC)
2. Click insert/edit image icon located on the bottom left of your tool bar in blackboard (next to insert file icon)
3. Click browse my computer
4. Click pictures
5. Locate your graph/picture
6. Click open

Using a Tool to Format APA Tables in SPSS Instructions:
 
In addition, here is some information on formatting tables in SPSS.  In case you are having difficulties within the class and having trouble creating APA tables, as well as have the default borders set.  Therefore, I created a file that you can upload into SPSS and allow all of your tables to appear as APA.  Here are the instructions on how to change default settings with SPSS into APA.  This method is much easier than the previous methods I have provided.  I hope these instructions help.
In SPSS

  • Go to Edit > Options
  • Go to the Pivot Tables tab
  • Download the file (APA-table look 1 1.stt) that I attached in announcements to your PC
  • Browse your PC and load the stt table look file into TableLook
  • Please choose “shrink width to fit”.
  • Press “Apply” at the bottom of the screen

Dear Doctoral Students,
As you would expect, assignments at Walden in each course are not designed to be duplicated in another course. Part of the purpose and design of a doctoral program is not only to prepare you to be an expert in your chosen area (primarily done via your capstone  project or dissertation), but to gain breadth, knowledge, skills and expertise in the many other areas of public health.
Thus, please advised that Walden does not permit re-use of your own scholarly work, including repeating a course, unless permission is granted in advance of your assignment submission by your instructor. The one exception is your dissertation, which can draw from your previous work.
Please review the Students’ Misuse of Their Own Scholarly Work section of our Academic Integrity Policy within our Student Code of Conduct.
Repeating a course due to a course failure/withdrawal – Permission is required from your instructor in advance before submitting work you completed in your previous attempt. Assignments that can only be written/answered in one way and without variation may be approved.
Submitting an assignment or part of an assignment from another course – This is not permitted unless the assignment specifically calls for it. Check with your instructor first.
Reuse of work within the same course – Assignments, such as a Scholar Practitioner Project, that build each week are permitted and not considered re-use of work. Using a discussion post for a later assignment should be approved by your instructor.

Week 1 Assignment Due Day 7 and by – June 7, 2020

Class,  Please remember that you do have an assignment this week in addition to the discussion assignment.
The assignment asks you to look at Variables, Measurement, and SPSS and hopefully you were able to down load the SPSS statistical software program.   Information is also provided regarding the download of SPSS and tutoring services to assist you in already posted announcements.
You will choose  one data set from two data sets that are provided.  Providing you have downloaded the SPSS, you should be able to just click on the data set you want to use and it will also open the SPSS software. You need to choose and open only one of the data sets (Afrobarometer dataset or the High School Longitudinal Study dataset). The data sets are found in this week’s Announcements/Doc Sharing and then choose two variables that interest you to complete this assignment from the data set you chose.
 

In addition, early in your Assignment, when you select which dataset you analyzed, please include the mean of the following variables depending on the data set you chose. If you are using the Afrobarometer Dataset, report the mean of Q1 (Age). If you are using the HS Long Survey Dataset, report the mean of X1Par1Edu.
 
But the questions below are for the two other variables that you chose from one of the data sets that I posted in announcements and doc sharing.:

 

 
You will be expected to write a 1- to 2-page summary and include a description of what each of the variables measure, the unit of analysis, and an explanation of the levels of measurement for each variable (i.e., nominal, ordinal, interval, ratio).  Please be sure to include implications for social change.  Additionally, please be sure to use proper APA formatting, which includes an APA title page. I look forward to reading your first assignment.

I am going to be your instructor for this course.
I want to encourage you to review the course syllabus and compare and plan out your weekly assignments to your existing calendars.  You will want to review your required activities early so that you will be able to stay caught up in this course.  In addition be sure to review the Student Handbook regarding Walden policies and procedures and to pay particular attention to completing your assignments on time.
Please, do not hesitate to contact me regularly using your Walden email.  We ask that you not use your personal email as many times it can get lost in spam.   Be sure to email me if you have any questions about the course or emergency situations which may cause problems in completing your assignments in a timely manner.

Re-authenticate SPSS Access

SPSS is a software package that provides students with statistical analysis, modeling, predictive, and survey research tools used in many of our courses and advanced research activities. Students can download the latest version of SPSS from the web free of charge. Walden is pleased to be able to continue offering this important resource free of charge for the duration of your program; and now the process for receiving SPSS is easier than ever.
Please note that all students will need to enter an updated access code on September 30, 2013 to re-authenticate their SPSS license. Read the instructions about updating an expired license. If you are in need of the latest version SPSS, version 20.0, you may download it by following the installation instructions.
Do I need to download the latest version?

  • Yes, if you have an older version and would like the current version.
  • Yes, if you had SPSS but your license has since expired and you need to continue using it.
  • Yes, if you are beginning your first course where you will be using SPSS.

  • Discussion Questions (DQ)

Initial responses to the DQ should address all components of the questions asked, including a minimum of one scholarly source, and be at least 250 words. Successful responses are substantive (i.e., add something new to the discussion, engage others in the discussion, well-developed idea) and include at least one scholarly source. One or two-sentence responses, simple statements of agreement or “good post,” and responses that are off-topic will not count as substantive. Substantive responses should be at least 150 words. I encourage you to incorporate the readings from the week (as applicable) into your responses.

  • Weekly Participation

Your initial responses to the mandatory DQ do not count toward participation and are graded separately. Part of your weekly participation includes viewing the weekly announcement and attesting to watching it in the comments.   • APA Format and Writing Quality

Familiarize yourself with the APA format and practice using it correctly. It is used for most writing assignments for your degree. Visit the Writing Center in the Student Success Center, under the Resources tab in Loud-cloud for APA paper templates, citation examples, tips, etc. Points will be deducted for poor use of APA format or absence of APA format (if required). Cite all sources of information! When in doubt, cite the source. Paraphrasing also requires a citation. I highly recommend using the APA Publication Manual, 6th edition.

  • Use of Direct Quotes

I discourage over-utilization of direct quotes in DQs and assignments at the Master’s level and deduct points accordingly. As Masters’ level students, it is important that you be able to critically analyze and interpret information from journal articles and other resources. Simply restating someone else’s words does not demonstrate an understanding of the content or critical analysis of the content. It is best to paraphrase content and cite your source.   • Late Policy

The university’s policy on late assignments is a 10% penalty PER DAY LATE. This also applies to late DQ replies. Please communicate with me if you anticipate having to submit an assignment late. I am happy to be flexible, with advance notice. We may be able to work out an extension based on extenuating circumstances. If you do not communicate with me before submitting an assignment late, the GCU late policy will be in effect. I do not accept assignments that are two or more weeks late unless we have worked out an extension. As per policy, no assignments are accepted after the last day of class. Any assignment submitted after midnight on the last day of class will not be accepted for grading.
