Assessing Community Health Care Needs
June, 2021
Community Health Assessment
A survey of the windshield is an essential way to collect information in a particular area (Villa Health, n.d.). These surveys are important because observers may examine a region and assess the community’s unmet requirements. A windshield survey helps the health sector design and carries through information of socio-economic factors including poverty levels, economic activities and community hazards to improve healthcare for a given population. (Villa Health, n.d.). The features and needs of each community are theirs. If you do not have a comprehensive understanding of these particular needs and their environment it may be difficult for practitioners to provide patient-oriented therapy. Notwithstanding its benefits, Windshield surveys are harmful because they favor quick decisions solely based on a field observation but do not grasp the reasons behind them. It can only be feasible for the erroneous impression to be based on physical assessments, so that changes cannot be initiated or carried out. This summary is intended to assess the health needs of Jordan and its cities.
The city of Jordan has a population of around 6,500 in urban and rural regions (Villa Health, n.d.). There are a lot of factors in the delivery of health care in this city. Jordan is home to two health institutions: St. Francis Health Center and Park Nicollet Health services. St. Francis Health Center offers emergency, ambulatory, and hospital health care services to local residents (Villa Health, n.d.). Also situated in this area, the Oak Terrace Senior Living Center provides a range of services, including independent, long-term and memory care (Villa Health, n.d.). The capital of Jordan has a downtown district with shops and public transportation for the lower to middle class. Unfortunately, none of the two health centers are near the city Center, therefore public transportation is an asset.
Executive Summary
Jordan is a town in the county of Scott, in the USA. The city’s many ethnic groupings include non-Hispanic whites, Hispanics, Native Hawaiians, African Americans, and Asians. Obesity and diabetes are two of Jordan City’s most prevalent health problems. Nevertheless, the city has social facilities and services well-equipped. The Jordan Community Education and Recreation Centre, for example, is home to Veteran’s Park. Two health facilities in the city, St. Francis Health Services and Oak Terrace Senior Living. All health facilities provide hospital, ambulatory, and emergency care.
Many religious groups, like the Methodist Church of Immanuel and the Lutheran Church of Hope, may provide spirits and additional medication via their prayers. In addition, many households have two cars on average. As a result, there is little usage of public transit. The development of lifestyle illnesses such as obesity may be explained.
General Condition and Public Health Needs
The city of Jordan has around 6,402 inhabitants. Among ethnic groupings of people are indigenous whites, Hispanics, Hawaiians, African Americans, and also consists of a good number of Asians. The most often known non-Hispanic is ethnic (68.4 percent), the least known being Asians (0.74 percent). (US BUREAU CENSUS, 2019). There are numerous religious institutions, which are mostly Christian denominations, according to the research that has been performed.
There are also many public and also private educational institutions in Jordan. The residential portions are highly maintained despite the absence of communities, featuring many roads, streets, boulevards and avenues. Many recreational facilities are also available, including parks and other public areas such as bookstores and pool parks. The street signs in the city have also altered to reflect the present political and social situation. The presence in the center and adjacent shopping centers, of empty buildings, is remarkably still intact and waiting to be inhabited by any company or business.
There are a number of major health centers, such as St Francis Health Services, located in Jordan City. In addition, because of a high median income of $61,592 and the poor poverty rate of 11.6%, many people have great access to health care (US CENSU BUREAU, 2018). However, all issues that individuals are facing include mental illness, heart disease, diabetes, stroke, cancer and obesity (Severtson et al. 2016).
Environmental Analysis
The health of the community is influenced by the physical environment. The health-related variables include the socio-economic status of citizens, the environment, type and nature of employment, social assistance networks, the kind and nature of professions, neighborhoods, and access to health facilities (Artiga, 2018). The interplay of these factors influences the health and disease-prone nature of a population.
Most Jordanian citizens earn $61,592 in their high family and are 11.6 percent poor (US CENSU BUREAU, 2018). The bulk of the population can provide most health care services due to their high family income. This includes identifying, treating, and managing early diseases.
In addition, health insurance is provided for the majority of urban inhabitants. On average, 1667 people visit primary care physicians per year, whereas there are 2477 dentists every year. In addition, 1088 people visit experts in mental health annually. The ratio between physician and patient is 1667 and 1. In addition, there are many health facilities in the city, including St. Francis Health Services in which the Nicollet Health Park and Allina Health Services are involved to provide treatment. The Valley View Health Center, Fairview Hospital and Queen of Peace Hospital are other facilities offering emergency treatment to Jordanians (Villa Health, n.d.).
A significant number of students live in Jordan City, which shows that there is a high level of education in the region. Examples of such educational institutions are: Jordan Middle School, Jordan High School and Jordan Elementary School, along with plans to construct Jordan’s first University. These institutions offer education and artistic services, sports, and out-of-school activities. In many of these schools’ students may pursue vocational courses. The University of Minnesota, Minnesota State University, and a number of smaller colleges belong to high schools in the city. Located in the center of town, the Jordan Public Library provides both educational and interlibrary books. These facilities generally indicate that people value and pay for education.
The Lagoon Park and the Jordan Community Education and Recreation Center are also for the Jordanian population, which are recreational facilities. Other recreational sites are the Minnesota River Conservatory, Minnesota Valley Animal Trail, and The Minnesota National Museum. In downtown Jordan there are also many restaurants, stores, breweries, and winegrowing grounds, all offering the inhabitants of Jordan a wonderful experience. There may therefore be excessive usage of alcohol in many vineyards and breweries which may result in heart and liver issues and other lifestyle conditions.
Conclusions
As shown in the condition of the streets of Jordan, the shopping plazas and the neighboring villages, the city is generally in good economic form. In addition, most institutions are above poverty level with an average annual familial return of 61,592 $ and a low poverty rate of 11.6%. (US CENSU BUREAU, 2018). In addition, the people in general are less exposed to environmental hazards since retail employment are a major concern. Only a tiny part of the population suffers health problems because of its outstanding architectural and physical environment and the availability of leisure activities. However, difficulties in lifestyle, especially among older people, are frequent.

All of our essay writing service products are 100% original, ensuring that there is no plagiarism in them. The sources are well-researched and cited so it is interesting. Our goal is to help as many students as possible with their assignments, i.e. our prices are affordable and services premium.
- Discussion Questions (DQ)
Initial responses to the DQ should address all components of the questions asked, including a minimum of one scholarly source, and be at least 250 words. Successful responses are substantive (i.e., add something new to the discussion, engage others in the discussion, well-developed idea) and include at least one scholarly source. One or two-sentence responses, simple statements of agreement or “good post,” and responses that are off-topic will not count as substantive. Substantive responses should be at least 150 words. I encourage you to incorporate the readings from the week (as applicable) into your responses. NURS-FPX6218 Leading the Future of Health Care
- Weekly Participation
Your initial responses to the mandatory DQ do not count toward participation and are graded separately. In addition to the DQ responses, you must post at least one reply to peers (or me) on three separate days, for a total of three replies. Participation posts do not require a scholarly source/citation (unless you cite someone else’s work). Part of your weekly participation includes viewing the weekly announcement and attesting to watching it in the comments. These announcements are made to ensure you understand everything that is due during the week.
- APA Format and Writing Quality
Familiarize yourself with the APA format and practice using it correctly. It is used for most writing assignments for your degree. Visit the Writing Center in the Student Success Center, under the Resources tab in Loud-cloud for APA paper templates, citation examples, tips, etc. Points will be deducted for poor use of APA format or absence of APA format (if required). Cite all sources of information! When in doubt, cite the source. Paraphrasing also requires a citation. I highly recommend using the APA Publication Manual, 6th edition.
- Use of Direct Quotes
I discourage over-utilization of direct quotes in DQs and assignments at the Master’s level and deduct points accordingly. As Masters’ level students, it is important that you be able to critically analyze and interpret information from journal articles and other resources. Simply restating someone else’s words does not demonstrate an understanding of the content or critical analysis of the content. It is best to paraphrase content and cite your source. NURS-FPX6218 Leading the Future of Health Care
- LopesWrite Policy
For assignments that need to be submitted to Lopes Write, please be sure you have received your report and Similarity Index (SI) percentage BEFORE you do a “final submit” to me. Once you have received your report, please review it. This report will show you grammatical, punctuation, and spelling errors that can easily be fixed. Take the extra few minutes to review instead of getting counted off for these mistakes. Review your similarities. Did you forget to cite something? Did you not paraphrase well enough? Is your paper made up of someone else’s thoughts more than your own? Visit the Writing Center in the Student Success Center, under the Resources tab in Loud-cloud for tips on improving your paper and SI score.
- Late Policy
The university’s policy on late assignments is a 10% penalty PER DAY LATE. This also applies to late DQ replies. Please communicate with me if you anticipate having to submit an assignment late. I am happy to be flexible, with advance notice. We may be able to work out an extension based on extenuating circumstances. If you do not communicate with me before submitting an assignment late, the GCU late policy will be in effect. I do not accept assignments that are two or more weeks late unless we have worked out an extension. As per policy, no assignments are accepted after the last day of class. Any assignment submitted after midnight on the last day of class will not be accepted for grading. NURS-FPX6218 Leading the Future of Health Care
- Communication
Communication is so very important. There are multiple ways to communicate with me: Questions to Instructor Forum: This is a great place to ask course content or assignment questions. If you have a question, there is a good chance one of your peers does as well. This is a public forum for the class. Individual Forum: This is a private forum to ask me questions or send me messages. This will be checked at least once every 24 hours.
