Impulsivity is the inclination to act upon sudden urges or desires without considering potential consequences; patients often describe impulsivity as living in the present moment without regard to the future (MentalHelp.net, n.d.). Thus, these disorders often manifest as negative behaviors, resulting in adverse outcomes for patients. For example, compulsivity represents a behavior that an individual feels driven to perform to relieve anxiety (MentalHelp.net, n.d.). The presence of these behaviors often results in addiction, which represents the process of the transition from impulsive to compulsive behavior.

In your role as the psychiatric nurse practitioner (PNP), you have the opportunity to help patients address underlying causes of the disorders and overcome these behaviors. For this Assignment, as you examine the client case study in this week’s Learning Resources, consider how you might assess and treat clients presenting with impulsivity, compulsivity, and addiction.

BACKGROUND

Mrs. Maria Perez is a 53 year old Puerto Rican female who presents today due to a rather “embarrassing problem.”

SUBJECTIVE

Mrs. Perez admits that she has had “problems” with alcohol since her father died in her late teens. She reports that she has struggled with alcohol since her 20’s and has been involved with Alcoholics Anonymous “on and off” for the past 25 years. She states that for the past 2 years, she has been having more and more difficulty maintaining her sobriety since the opening of the new “Rising Sun” casino near her home. Mrs. Perez states that she and a friend went to visit the new casino during its grand opening at which point she was “hooked.” She states that she gets “such a high” when she is gambling. While gambling, she “enjoys a drink or two” to help calm her during high-stakes games. She states that this often gives way to more drinking and more reckless gambling. She also reports that her cigarette smoking has increased over the past 2 years and she is concerned about the negative effects of the cigarette smoking on her health.

She states that she attempts to abstain from drinking but she gets such a “high” from the act of gambling that she needs a few drinks to “even out.” She also notices that when she drinks, she doesn’t smoke “as much,” but she enjoys smoking when she is playing at the slot machines. She also reports that she has gained weight from drinking so much. She currently weights 122 lbs., which represents a 7 lb. weight gain from her usual 115 lb. weight.

Mrs. Perez is quite concerned today because she borrowed over $50,000 from her retirement account to pay off her gambling debts, and her husband does not know.

MENTAL STATUS EXAM

The client is a 53 year old Puerto Rican female who is alert and oriented to person, place, time, and event. She is dressed appropriately for the weather and time of year. Her speech is clear, coherent, and goal directed. Her eye contact is somewhat avoidant during the clinical interview. When you make eye contact with her, she looks away or looks down. She demonstrates no noteworthy mannerisms, gestures, or tics. Her self-reported mood is “sad.” Affect is appropriate to content of conversation and self-reported mood. She denies visual or auditory hallucinations, and no delusional or paranoid thought processes are readily appreciated. Insight and judgment are grossly intact; however, impulse control is impaired. She is currently denying suicidal or homicidal ideation.

Guidance to Student

Anxiety is a common side effect of Vivitrol. Mrs. Perez reports that she is doing well with this medication, and like other side effects, the anxiety associated with this medication may be transient. The psychiatric mental health nurse practitioner should never initiate benzodiazepines in a client who already has issues with alcohol, or other substance dependencies. Additionally, benzodiazepines are not to be used long-term. Problems associated with long-term benzodiazepine use include the need to increase the dose in order to achieve the same therapeutic effect. This is what we are seeing in Mrs. Perez’s case.

The most appropriate course of action in this case would be to continue the current dose of Vivitrol, while decreasing the Valium with the goal of discontinuation of the drug within the next two weeks. At that point, you would need to evaluate whether or not the side effect of anxiety associated with Vivitrol persists.

Increasing the dose of Valium would not be appropriate, neither would maintaining her on the current dose of Valium. Additionally, the client should be referred for counseling to help with her gambling addiction, as there are no FDA approved medications gambling disorder.

Medication should never be added treat side effect of another medication, unless that side effect is known to be transient (for instance, benzodiazepines are sometimes prescribed to overcome the initial problem of “activation” associated with initiation of SSRI, or SNRI therapy). However, in a client with multiple addictive disorders, benzodiazepines should never be used (unless they are only being used for a limited duration of therapy such as acute alcohol detoxification to prevent seizures).

Additionally, it should be noted that Mrs. Perez continues to engage in problematic gambling, at considerable personal financial cost. Mrs. Perez needs to be referred to a counselor who specializes in the treatment of gambling disorder, and should also be encouraged to establish herself with a local chapter of gamblers anonymous.

You need to discuss smoking cessation options with Mrs. Perez in order to address the totality of addictions, and to enhance her overall health.

Assessing and Treating Clients with Dementia

The treatment of brain degeneration illnesses such as Alzheimer’s will require the PMHNP to consider the benefits and side effects of such treatments. The PMHNP will recognize that though the brain degeneration caused by conditions such as Alzheimer’s is not reversible, it can be controlled when the right medication and dosage are administered to patients. This paper explores the pharmacokinetics and pharmacodynamics of the medications administered to control Alzheimer’s in a 76-year-old Iranian male.

Decision #1

For the first decision, I decided to administer Exelon (rivastigmine) 1.5 mg orally BID with an increase to 3 mg orally BID after two weeks. I made the decision to administer Exelon (rivastigmine) 1.5 mg to the patient and increase the dosage to 3 mg after two weeks as the drug is known to be effective in limiting the symptoms of degenerative brain disease as a cholinesterase inhibitor (Khoury, Rajamanickam, & Grossberg, 2018). I also made the decision, to begin with, a 1.5 mg dosage and increase to 3mg after two weeks as this would help to avoid severe side effects on the patients a such as the loss of appetite, nausea, and upset stomach (Khoury et al., 2018). The medication would benefit the patients and cause less harm.

The other two decisions, which would be to administer Aricept (donepezil) 5 mg orally or Razadyne (galantamine) 4 mg orally BID, would be less advantageous and would not be considered.  The central reason for not administering Aricept (donepezil) 5 mg to the 76-year-old Iranian male would be because of the severe side effects associated with the drug. According to Agboton, Mahdavian, Singh, et al. (2014), Aricept has been shown to cause a slow and irregular heartbeat in some patients, which can result in fainting. On the other hand, the decision to administer Razadyne would be less advantageous as it can cause severe side effects such as fast, slow, and irregular breathing, which would be detrimental to the elderly patient’s health. Razadyne would also have a higher probability of contributing to severe side effects such as slow or irregular heartbeats in a patient and would not be considered (Nakagawa, Ohnishi, Kobayashi, et al., 2017).

By making the decision to administer Exelon (rivastigmine) 1.5 mg orally and increase to 3 mg orally BID in 2 weeks, I was hoping to help limit the symptoms of Alzheimer’s on the patient, including loss of interest in important activities, and increasing the patients Mini-Mental State Exam (MMSE) score. According to Kandiah, Pai, Senanarong, et al. (2017), Exelon will be effective in treating most symptoms of Alzheimer’s and will boost the brain’s activity of patients.

What I was hoping to achieve after administering Exelon 1.5 mg orally and increasing the dose to 3 mg orally after two weeks and the results of the decision were different because no improvements were noted in the patient’s MMSE score. The patient was also quite disinterested in important activities in their life, such as religious activities. From the results, it was evident that the medication had not yet been effective on the patient. According to Khoury et al. (2018), cholinesterase inhibitors will not be effective from the onset, with effects likely to be observed in the second month of administration.

Decision #2

For the second decision, I choose to increase the dosage of Exelon to 4.5 mg orally. I selected this decision because the patient had not displayed any severe side effects with the administration of 3mg of Exelon in the past month. Increasing the dosage of Exelon to 4.5 mg was also needed to bring about the preferred results of limiting the symptom of Alzheimer’s on the patient. According to Kandiah et al. (2017), increasing the dosage of Exelon is recommended among patients who do not experience severe side effects so as to limit the symptoms of Alzheimer’s.

By making the decision to increase the dosage of Exelon to 4.5 mg, I was hoping to improve the patient’s brain functionality and interest in activities which they liked. What I expected to achieve in increasing the dosage of Exelon to 4.5 mg was similar to what I achieved in that after a month, the patients showed increased functionality by attending religious activities and showing interest. This showed the dosage was effective in limiting the symptoms of Alzheimer’s on the patient.

Decision #3

For the third decision, I choose to increase Exelon dosage to 6mg orally. According to Khoury et al. (2018), increasing the dosage of Exelon is recommended in cases where the patient does not show any severe side effects. The increased dosage will be more effective in limiting Alzheimer’s symptoms.

By making the decision to increase the Exelon dosage of the 76-year-old Iranian patient to 6mg, I was hoping to sustain the patient’s interests in activities which they liked and improve the brain functionality. Cholinesterase inhibitors help to stabilize the symptoms of Alzheimer’s and help patients to regain interest in activities they consider important while functioning better (Kandiah et al., 2017). What I was hoping to accomplish by increasing the dosage was similar to what I achieved in that the patient showed an increased interest in religious activities and general improvement in brain function. This showed that the increased dosage was effective in limiting Alzheimer’s symptoms in the patients.

Ethical Considerations

In treating the 76-year-old Iranian male, the ethical principles of confidentiality and beneficence would be followed. Only the medication that provides the benefits to the patients would be considered, with all the information related to the treatment being held in confidence by all involved parties.

Assessing and Treating Clients with Dementia

The treatment of brain degeneration illnesses such as Alzheimer’s will require the PMHNP to consider the benefits and side effects of such treatments. The PMHNP will recognize that though the brain degeneration caused by conditions such as Alzheimer’s is not reversible, it can be controlled when the right medication and dosage are administered to patients. This paper explores the pharmacokinetics and pharmacodynamics of the medications administered to control Alzheimer’s in a 76-year-old Iranian male.

Decision #1

For the first decision, I decided to administer Exelon (rivastigmine) 1.5 mg orally BID with an increase to 3 mg orally BID after two weeks. I made the decision to administer Exelon (rivastigmine) 1.5 mg to the patient and increase the dosage to 3 mg after two weeks as the drug is known to be effective in limiting the symptoms of degenerative brain disease as a cholinesterase inhibitor (Khoury, Rajamanickam, & Grossberg, 2018). I also made the decision, to begin with, a 1.5 mg dosage and increase to 3mg after two weeks as this would help to avoid severe side effects on the patients a such as the loss of appetite, nausea, and upset stomach (Khoury et al., 2018). The medication would benefit the patients and cause less harm.

The other two decisions, which would be to administer Aricept (donepezil) 5 mg orally or Razadyne (galantamine) 4 mg orally BID, would be less advantageous and would not be considered.  The central reason for not administering Aricept (donepezil) 5 mg to the 76-year-old Iranian male would be because of the severe side effects associated with the drug. According to Agboton, Mahdavian, Singh, et al. (2014), Aricept has been shown to cause a slow and irregular heartbeat in some patients, which can result in fainting. On the other hand, the decision to administer Razadyne would be less advantageous as it can cause severe side effects such as fast, slow, and irregular breathing, which would be detrimental to the elderly patient’s health. Razadyne would also have a higher probability of contributing to severe side effects such as slow or irregular heartbeats in a patient and would not be considered (Nakagawa, Ohnishi, Kobayashi, et al., 2017).

By making the decision to administer Exelon (rivastigmine) 1.5 mg orally and increase to 3 mg orally BID in 2 weeks, I was hoping to help limit the symptoms of Alzheimer’s on the patient, including loss of interest in important activities, and increasing the patients Mini-Mental State Exam (MMSE) score. According to Kandiah, Pai, Senanarong, et al. (2017), Exelon will be effective in treating most symptoms of Alzheimer’s and will boost the brain’s activity of patients.

What I was hoping to achieve after administering Exelon 1.5 mg orally and increasing the dose to 3 mg orally after two weeks and the results of the decision were different because no improvements were noted in the patient’s MMSE score. The patient was also quite disinterested in important activities in their life, such as religious activities. From the results, it was evident that the medication had not yet been effective on the patient. According to Khoury et al. (2018), cholinesterase inhibitors will not be effective from the onset, with effects likely to be observed in the second month of administration.

Decision #2

For the second decision, I choose to increase the dosage of Exelon to 4.5 mg orally. I selected this decision because the patient had not displayed any severe side effects with the administration of 3mg of Exelon in the past month. Increasing the dosage of Exelon to 4.5 mg was also needed to bring about the preferred results of limiting the symptom of Alzheimer’s on the patient. According to Kandiah et al. (2017), increasing the dosage of Exelon is recommended among patients who do not experience severe side effects so as to limit the symptoms of Alzheimer’s.

By making the decision to increase the dosage of Exelon to 4.5 mg, I was hoping to improve the patient’s brain functionality and interest in activities which they liked. What I expected to achieve in increasing the dosage of Exelon to 4.5 mg was similar to what I achieved in that after a month, the patients showed increased functionality by attending religious activities and showing interest. This showed the dosage was effective in limiting the symptoms of Alzheimer’s on the patient.

Decision #3

For the third decision, I choose to increase Exelon dosage to 6mg orally. According to Khoury et al. (2018), increasing the dosage of Exelon is recommended in cases where the patient does not show any severe side effects. The increased dosage will be more effective in limiting Alzheimer’s symptoms.

By making the decision to increase the Exelon dosage of the 76-year-old Iranian patient to 6mg, I was hoping to sustain the patient’s interests in activities which they liked and improve the brain functionality. Cholinesterase inhibitors help to stabilize the symptoms of Alzheimer’s and help patients to regain interest in activities they consider important while functioning better (Kandiah et al., 2017). What I was hoping to accomplish by increasing the dosage was similar to what I achieved in that the patient showed an increased interest in religious activities and general improvement in brain function. This showed that the increased dosage was effective in limiting Alzheimer’s symptoms in the patients.

Ethical Considerations

In treating the 76-year-old Iranian male, the ethical principles of confidentiality and beneficence would be followed. Only the medication that provides the benefits to the patients would be considered, with all the information related to the treatment being held in confidence by all involved parties.

BACKGROUND INFORMATION

The client is a 46-year-old white male who works as a welder at a local steel fabrication factory. He presents today after being referred by his PCP after a trip to the emergency room in which he felt he was having a heart attack. He stated that he felt chest tightness, shortness of breath, and feeling of impending doom. He does have some mild hypertension (which is treated with low sodium diet) and is about 15 lbs. overweight. He had his tonsils removed when he was 8 years old, but his medical history since that time has been unremarkable. Myocardial infarction was ruled out in the ER and his EKG was normal. Remainder of physical exam was WNL.

He admits that he still has problems with tightness in the chest and episodes of shortness of breath- he now terms these “anxiety attacks.” He will also report occasional feelings of impending doom, and the need to “run” or “escape” from wherever he is at.

In your office, he confesses to occasional use of ETOH to combat worries about work. He admits to consuming about 3-4 beers/night. Although he is single, he is attempting to care for aging parents in his home. He reports that the management at his place of employment is harsh, and he fears for his job. You administer the HAM-A, which yields a score of 26.

Client has never been on any type of psychotropic medication.

MENTAL STATUS EXAM

The client is alert, oriented to person, place, time, and event. He is appropriately dressed. Speech is clear, coherent, and goal-directed. Client’s self-reported mood is “bleh” and he does endorse feeling “nervous”. Affect is somewhat blunted, but does brighten several times throughout the clinical interview. Affect broad. Client denies visual or auditory hallucinations, no overt delusional or paranoid thought processes readily apparent. Judgment is grossly intact, as is insight. He denies suicidal or homicidal ideation.

Guidance to Student At this point, it may be appropriate to continue client at the current dose. It is clear that the client is having a good response (as evidenced by greater than a 50% reduction in symptoms) and the client is currently not experiencing any side effects, the current dose can be maintained for 12 weeks to evaluate full effect of drug. Increasing drug at this point may yield a further decrease in symptoms, but may also increase the risk of side effects. This is a decision that you should discuss with the client. Nothing in the client’s case tells us that we should consider adding an augmentation agent at this point as the client is demonstrating response to the drug. Avoid polypharmacy unless symptoms cannot be managed by a single drug.

The first question I would ask this patient is whether she has sought grief counseling? The second would be do you consume excess caffeine? And the third would be are you compliant with taking your Zoloft? I would seek feedback from her family and friends to find out if her habits or behavior has changed in anyway?

The differential diagnosis that I would suggest would be grief. Bereavement often effects individuals both physically and mentally. Severe, persistent and disabling grief known as complicated grief (CG) as well as prolonged grief disorder (PGD) have been added to the International Classification of Diseases 11th Edition (Lancel et al., 2020). Hallmark presentation of PGD are persistent and severe yearning for the deceased and/or preoccupation with the deceased, combined with symptoms of intense emotional pain. Bereavement and grief are associated with sleep disturbances. Conversely, the lack of sleep often exacerbates or prolongs the grief process.

I would have a CMP drawn to assess overall hemodynamic status. I would perhaps order a sleep study to measure the true amount of sleep that the patient is getting and assess any physical causes that may be disturbing her sleep. I would be cautious about using sleep aids due to the patient’s age and risk for falls.

The two medications that I would add would be:

Trazadone 50mg po HS, may repeat in 1hr if remains awake. Trazadone is a commonly used atypical antidepressant with strong sedative actions. Known to decrease sleep latency and prolong sleep duration; it does not cause tolerance or physical dependence (Rosenthal & Burchum,2021). I would monitor for postural hypotension and caution the patient that she may experience some daytime grogginess.

To avoid the use of another psychotropic, I would choose to begin the patient on a complementary therapy using a medicine which combines melatonin, vitamin B6 and extracts from medicinal plants California poppy extract, passionflower extract, and lemon balm extract. Novanuit Triple Action is associatied with high compliance, a benign side effect profile, and low potential for dependence and addiction (Lemoine, 2019). This product has been proven in a pilot study to significantly increase elements of sleep such as total sleep duration, decrease sleep onset latentcy and daytime impairment related to disturbed sleep pattern. During the drug trial, subjects used a daily electronic sleep diary to measure sleep quality, total sleep duration and functional daytime disability. After 2 weeks of consistent use of Novanuit, sleep was rated to increase 2 points higher on a 0-10 scale. After 4 weeks, compliance was measured at 97.5%, with reports of significant improvement of sleep and mood.

I would follow-up with the patient at 2 weeks to assess improvement of sleep quality and duration. I would continue to encourage grief counseling. I would also encourage light exercise and participating in enjoyable activities. I would also assess patient’s depressive symptoms and consider increasing her Zoloft to 150mg if depression has not improved with better sleep.

I would schedule an appointment 4 weeks to check patient’s progress.

Treatment for a Patient with a Common Condition

Insomnia disorder in older adults is multifactorial and is associated with difficulty falling asleep or staying asleep and can also be implicated in many medical and psychiatric disorders. Insomnia is a disruption in the amount and quality of sleep that impairs functioning (Morgan & Townsend, 2021). As people age, sleep patterns and the amount of sleep can also change. A baby can sleep between 10 to 14 hours per day, whereas an older person is recommended to rest for at least 7-8 hours daily (Brewster et al., 2018). Insomnia is frequently associated with aging, related to decreased restorative stage 3 of nonrapid eye movement sleep and advance phase shift leading to early morning riser (Cherukuri et al. 2018). In the case scenario of this 75-year-old patient that already has several comorbidities of HTN, DM, & major depressive disorder (MDD), who had experienced the death of her spouse of 41 years ten months ago is a significant life event that is a risk factor of her mood and her current state of insomnia. While evaluating the current patient list of medication, she is on Sertraline 100 mg daily for her MDD. One of the side effects of that drug is difficulty falling asleep or staying asleep (U.S. Food & Drug Administration n.d).

There are many pharmacological, non-pharmacological that can be used to provide appropriate intervention for insomnia and tools used to assess insomnia, but the clinician should be cautious when evaluating an older adult due to the possibility of polypharmacy. Three questions I might ask this patient. Using an insomnia questionnaire includes:

How often do you have trouble sleeping, and when did insomnia begin?

How long does it take to fall asleep?

How often do you awaken during the night, and how long does it take to fall back to sleep? And I would even go further to ask the patient what had she tried to help improve her insomnia?

Using open-ended questions can help differentiate the type of insomnia, either acute, which is often caused by an emotional or physical discomfort such as a life stressor (DerSarkissian, 2019), in this patient situation, the passing of husband of 41 years, chronic insomnia, or pharmacological can all be a factor. Therefore, as a clinician asking her about her sleep history should be evaluated to help guide treatment. Identifying other people in the patient life that can give feedback is vital; using a patient and family-centered approach is best. If they live in the same house, the patient children can be helpful, but if they do not live in the same place, it might be hard to ask some questions regarding sleeping habits, such as if they snore or have restless leg syndrome.

Unlike other sleep disorders, insomnia disorder relies on self-report (Levenson et al., 2015). The sleep disorder test is often used to diagnose insomnia and a physical exam where the clinician assesses the patient’s sleep history to determine the cause of the problem (DerSarkissian, 2019). Other diagnostic tests that may be appropriate in this patient’s case would be to have her keep a sleep diary, and it can help the clinician tracking the sleep patterns; another test is an Epworth Sleeping Scale that can be used to assess her daytime sleepiness as stated by DerSarkissian, 2019. Although this patient physical exam is within the defined limit and she denies any suicidal ideation, I think a physical and mental health exam is warranted to re-evaluate her mental status. I would recommend that the patient continues with her current Sertraline 100 mg daily, but take it early in the morning and use other technique mechanisms that can help her fall asleep and return in 4 weeks.

Differential Diagnosis

A differential diagnosis is grief related to the life stressors of her husband passing, is one the diagnosis that can be applied appropriately. Two pharmacological agents I would recommend are Ambien 5 mg for women by mouth at bedtime and in older individuals like this 75-year-old patient. Restoril 7.5 mg orally once a day at bedtime is the initial therapy recommended until the individual response is determined (Sinha, 2021).

In conclusion, proper evaluation and assessment of patient insomnia can help the clinician effectively develop mechanisms and strategies for managing insomnia. As a future nurse practitioner, the main goal is to help the patient manage and cope with life stressors to help improve her insomnia.

Delusional Disorders: Pakistani Female With Delusional Thought Processes

 

BACKGROUND

The client is a 34-year-old Pakistani female who moved to the United States in her late teens/early 20s. She is currently in an “arranged” marriage (her husband was selected for her when she was 9 years old). She presents following a 21-day hospitalization for what was diagnosed as “brief psychotic disorder.” She was given this diagnosis as her symptoms have persisted for less than 1 month.

Prior to admission, she was reporting visions of Allah, and over the course of a week, she believed that she was the prophet Mohammad. She believed that she would deliver the world from sin. Her husband became concerned about her behavior to the point that he was afraid of leaving their 4 children with her. One evening, she was “out of control,” which resulted in his calling the police and her subsequent admission to an inpatient psych unit.

During today’s assessment, she appears quite calm and insists that the entire incident was “blown out of proportion.” She denies that she believed herself to be the prophet Mohammad and states that her husband was just out to get her because he never loved her and wanted an “American wife” instead of her. She says she knows this because the television is telling her so.

She currently weighs 140 lbs., and she is 5’ 5.

SUBJECTIVE

Client reports that her mood is “good.” She denies auditory/visual hallucinations but believes that the television talks to her. She believes that Allah sends her messages through the TV. At times throughout the clinical interview, she becomes hostile towards you but then calms down.

A review of her hospital records shows that she received a medical workup from physician, who reported her to be in overall good health. Lab studies were all within normal limits.

Client admits that she stopped taking her Risperdal about a week after she got out of the hospital because she thinks her husband is going to poison her so that he can marry an American woman.

MENTAL STATUS EXAM

The client is alert and oriented to person, place, time, and event. She is dressed appropriately for the weather and time of year. She demonstrates no noteworthy mannerisms, gestures, or tics. Her speech is slow and, at times, interrupted by periods of silence. Self-reported mood is euthymic. Affect is constricted. Although the client denies visual or auditory hallucinations, she appears to be “listening” to something. Delusional and paranoid thought processes as described above. Insight and judgment are impaired. She is currently denying suicidal or homicidal ideation.

You administer the PANSS which reveals the following scores:

-40 for the positive symptoms scale

-20 for the negative symptom scale

-60 for general psychopathology scale

Start Zyprexa (olanzapine) 10 mg orally at BEDTIME

RESULTS OF DECISION POINT ONE

  • Client returns to clinic in four weeks
  • Her PANSS decreases to a partial response (decrease in positive symptoms by 25%)
  • She comes in today with a reported weight gain of 5 pounds. When questioned further on this point, she states that she can never seem to get full from her meals, so she is snacking constantly throughout the day

Decision Point Two

Decrease Zyprexa to 7.5 mg orally at BEDTIME

RESULTS OF DECISION POINT TWO

  • Client returns to clinic in four weeks
  • Result of next decision (what happened): Client worsens (her positive symtpoms scale increased by 25% and her negative symptom scale score decreased by 10% indicating improved negative symptoms), but her weight becomes stabilized and excessive hunger abates
  • Husband explains that she is becoming less manageable at home, and he has to take time off from work because he is fearful of leaving her alone

Decision Point Three

Increase Zyprexa 10 MG orally at BEDTIME

Guidance to Student
Weight gain is a significant problem with Zyprexa. Next to Clozaril (clozapine), Zyprexa causes the most weight gain of all the atypical antipsychotics. This is a side effect that a significant number of clients will experience. There also appears to be an increased association of newly diagnosed diabetes mellitus in clients treated with Zyprexa. Although this can be disease related in this population, Zyprexa is above what would be considered coincidental.
Risperdal is a good option, although it is dosed twice daily and compliance in this population can be problematic. There is evidence that shows giving Risperdal all at once can be efficacious and therefore could be an option down the road should compliance become an issue. Weight gain is also possible with Risperdal, but it is not as great as that seen with Zyprexa. If compliance does become an issue with this client, Risperdal has a long-acting injectable formulation, Risperdal Consta, that could be used. Remember, Risperdal Consta has to be given every 2 weeks at the provider’s office, and therapeutic blood levels take time to achieve (on average 3–6 weeks or 2–3 injections). Oral overlapping therapy is required to bridge this period of time. Another option in someone who responds to Risperdal would be Invega Sustenna (paliperidone palmitate), which is the first metabolite of Risperdal and has greater activity at the D2 receptor than Risperdal. An advantage of Invega Sustenna over Risperdal Consta is that therapeutic blood levels are attained within the first 4–7 days, and overlapping oral therapy is usually not necessary. A disadvantage is that during the initiating phase of medication, the first two doses need to be given within 4–7 days of one another. This is followed by monthly injections. There is another product on the market called Invega Trinza, which is given once every 3 months. This product is for clients who have been stabilized on Invega Sustenna for at least 4 months where the last two doses were the same strength (two months of 156 mg injections).
Increasing Zyprexa to 15 mg at bedtime will only worsen the weight gain side effect. While additional benefits from increasing the dose may be possible from an efficacy standpoint, side effects always need to be taken into consideration. “First, do no harm.” Qsymia is a weight loss medication that is a combination of phentermine and topiramate. It is only indicated to treat obesity. This client’s BMI (28.9 kg/M2) does not fit the definition of obesity (BMI >30 Kg/M2- Following from CDC website: Class 1: BMI of 30 to < 35, Class 2: BMI of 35 to < 40, Class 3: BMI of 40 or higher. Class 3 obesity is sometimes categorized as “extreme” or “severe” obesity). There are two things wrong with this therapy option. First, there are only a few occasions where add-on therapy to treat a side effect is acceptable, and weight gain is not one of those scenarios. Secondly, phentermine has a lot of cardiovascular toxicities (such as elevated BP, HR, and increased workload on the heart).

Initial responses to the DQ should address all components of the questions asked, including a minimum of one scholarly source, and be at least 250 words. Why is this important?

Almost 6,800 natural disasters occur worldwide each year (Homeland Security, 2021).

Following natural disasters, one-third to one-half of the citizens in the area are affect by mental health disparities (Cartier, 2021).

There are few immediate resources available following a natural disaster.

Major Findings Of the articles, there were two cross-sectional studies, one quasi-experimental study, a planned cohort study, and a systematic review.

1 This study was conducted to determine if the number of traumatic events experienced by the residents of Fort McMurray correlated with the amount of mental health despair in the community. The results showed multiple traumas such as wildfires, trauma, and the COVID-19 pandemic contributed to a much higher likelihood of experiencing anxiety, depression, or PTSD. Agyapong et al., 2022
2 In 2018, Texas residents were invited to complete and online survey of the effects of Hurricane Harvey and the traumatic flooding. In 2020, there was a nationwide survey sent out to residents to determine the level of disparity experienced by residents in the pandemic. The results show that the pandemic layered on to the experience of flooding from Hurricane Harvey greatly increased the mental health disease among the citizens. Callender et al., 2022
3 This was a cross-sectional analysis completed from citizens that were affected by flooding from December 2013-March 2014. All citizens in the area were invited to participate in the surveys, and the participants were categorized in to flooded, disrupted by flooding, or unaffected. The results showed highest anxiety among participants that were flooded, but there was also a significant level of anxiety experienced by the disrupted lives among the participants. Waite et al., 2017
4 The English National Study on Flooding and Health collected information from houses in the community that were flooded, disrupted, or unaffected by flooding. The results showed that women were more likely to suffer from depression, anxiety, and PTSD especially from losing items of sentimental value. For men, there was more concern over relationship problems and possible depression. Tempest et al., 2017
5 The authors created a conceptual behavioral framework for individuals that suffered from depression, anxiety, or PTSD following a traumatic event. There were in-person, web-based, and telephone interviews conducted to determine if the framework was effective in determining immediate need for mental health resources. The results showed the framework was very effective in assessing the individual to assist in immediate need. Goldmann et al., 2021

Low sample size; No generalizability to the population

There was no updated surveys/questionnaires sent to the individuals to determine the long-term effects of the individuals

Some individuals were unable to be reached due to the lack of resources following the disasters
