Children and adolescents face stigma/taboo when they have a mental disorder (the University of South California, n.d.). There is a shortage of education and health care for the underage with mental health disorders (World Health Organization, n.d.). That being said, one of Healthy People 2020’s objectives is to “increase the proportion of children with mental health problems who receive treatment” (Healthy People 2020, n.d.). Therefore, funding is needed for further research and treatment programs (University of South California, n.d.).

Describe the administrative agenda focus related to this issue for the current and two previous presidents. Trumps administration signed the “SUPPORT for Patients and Communities Act.” This act “expands access to inpatient treatment and other evidence-based options, for those with mental illness.” (The White House, 2019). The Department of Health and Human Services approved Medicaid to support treatment options for those with a mental illness and emotionally disturbed children (White House, 2019). In 2013, President Obama ordered to make mental health access for children and adolescents a priority. By increasing resources to schools, ensure treatment needed for the children and adolescents and guarantee that health insurance covers mental health (Gardner, 2013). President Bush’s agenda for mental health was to help eliminate the stigma seen in those who are mentally ill
(Lenzer, 2005).
Identify the allocations of financial and other resources that the current and two previous presidents dedicated to this issue. President Trump increased the budget to $723 million for the Community Mental Health Block Grant. The President also added fund specifically to the Children’s Mental Health Services. (Pellit, 2019) President Obama funded $50 million to community health centers to increase accessibility of services to those in need of mental health and/or substance abuse assistance. (Ingoglia, 2013) President Bush funded $126.2 to the Department of Health and Human Services, these funds were spread to provided assistance to services and activities including mental health and substance abuse. (The White House, 2001)
Explain how each of the presidential administrations approached the issue. President Trump goal is to open additional psychiatric institutes as well as providing mental health services. (Craig, 2019). Furthermore, President Trump wants to help focus on helping understand mental health issues as well as reducing the effects of mental health in the community. (The White House, 2019) President Obama proposed a new program to train adults, most importantly, teachers, in ways to detect and how to intervene to mental illness in children and adolescents. This project is called Advancing Wellness and Resilience in Education AWARE. (Szabo, 2013) President Bush approached this issue by signing Mental Health Parity and Addictions Equity Act of 2008, this law makes health insurance provide coverage equally to those who use insurance for mental health and substance abuse as those with a medical health issue. (Hart, 2016)

There is no doubt that money and economics makes things move in our society today. Even churches, charitable organizations, and humanitarian groups need money and funding to stay open. Healthcare is no exception, there is an economic engine that drives deliverance of healthcare, and as our text puts it graciously, “Similarly, the healthcare market as viewed by economists is amoral: When confronted with finite resources, there will be losers and winners. This is a tough concept for nurses to swallow” (Mistead, 2016, p. 285).
This week’s discussion is about the turbulent relationship between cost and care. As a registered nurse who have taken care of numerous patients, I wish nothing but the best for my patients far as their health and wellbeing. Though I would like to say that healthcare has strengthen in the humanistic position outlined by the Nightingale pledge, the romanticized ideals of healthcare is diminishing, and the postmodern technology and economics driven engine is now driving healthcare towards its future.
In a simple model of economics, supply and demand curve is often used to teach basic principles of economy. One of the many factors of growing prices of healthcare is that demand is greater than the supply (Knickman & Kovner, 2015, p. 219). In the basic goods and services model, when demand rises, several things happen: first prices tend to go up, two the free market sees an opportunity to meet supply (thus make profit) so competition will supply the demanded goods and services, and lastly prices will stabilize once demand is met.
In the healthcare market, demands for healthcare services provided is met by hospitals, clinics, and other ancillary specialties that required trained and licensed professionals (Knickman & Kovner, 2015, p. 214-215). However, of course, the healthcare market does not behave in the standard economic behavior, for supply is “constrained by licensure and educational requirements (Knickman & Kovner, 2015, p. 2019). Registered nurses, by far, comprise of the largest portion of the health care occupations in the United States (Knickman & Kovner, 2015, p. 215), and just like any supply chain, it too can be affected by economic upwind and downturns.
In a narrative review on nursing labor markets during economic downturns, Alameddine and his colleagues reviewed the implications of quality of care in healthcare when the nursing labor forces froze or decreased (Alameddine et al., 2012). The paper recommended to larger healthcare employees and governments that adequately stabilizing and even increasing the nursing workforce during an economic downturn lead to stabilization of providence of healthcare and increased quality of care (Alameddine et al., 2012). This is easier said than done.
The Washinton Post review by Rob Stein, regarding prostate drug Provenge, brings in light the controversy that occurred when Medicare officials reviewed if the cost versus benefit of Provenge, in short, whether to pay for it or not (Stein, 2010). The debate on prices for essential medication has been in the topic of healthcare debate recently, to include Epipen and insulin price hike by pharmaceuticals. This bring up the issue of FDA providing patents for companies to come up with new drugs, essentially creating a monopoly in the market place, until the patent period runs out and generics can be manufactured, a 20-year time frame (Pauly, 2018). In an economic sense, is paying a million dollars for a short prolongation of quality of life worth it? In my opinion, it is not, but I realize that subjectively, 2 to 4 years with a loved one is invaluable for an individual, but objectively costly for society.
There is a consensus that our medical care cost and delivery is inefficient and needs to be improved (Pauly, 2018). To me, there is not silver bullet, for healthcare is a divers and complex organism that require a holistic, multidisciplinary approach to solve for its inefficiency and waste. I believe that by discussing thoroughly, with personal biases aside, is the key to making compromise at all level, in which we can finally benefit society as a whole.

Politics and the Patient Protection and Affordable Care Act
The divisive politics and polarization have made it difficult for American leaders to rethink about the much-anticipated reforms in the healthcare sector. Some argue that the current affordable healthcare program under the Affordable Care Act is economical. From a cost benefit analysis approach, for a program to be economical, the benefits must be high compared to the amount incurred in the implementation of the program (Milstead & Short, 2019). The Act makes it possible for most of the citizens to access healthcare services and, therefore, repealing it may make legislators lose political support.
The politics around the affordable healthcare Act is that if it is repealed, some political leaders fear that many people will lose the insurance cover (Hagan et al., 2017). There is also fear that the repeal of the Act will result in a massive loss of jobs with the reduction of people who have access to the insurance cover. For instance, if the Act is repealed, many people who acquired their insurance coverage under the affordable healthcare program will automatically lose them. Millions of low-income earners who got covered under the Medicaid program will lose them, and consequently, many Americans will end up uninsured. This will affect their ability to access and afford healthcare services.
From Feldstein’s point of view, it is unlikely that the politicians will support the repeal of the Affordable care Act. This is based on the fact that from a cost-benefit analysis, the politicians are only likely to support a legislative agender that has more beneficial to them (Feldstein, 2006). On this basis, they are not likely to repeal the Affordable Care Act because it will not be beneficial to the Americans. This is reflected by the fact that despite the efforts by President Trump’s efforts to have the Act repealed, he has not been succeeded. For instance, even when the republics were in the control of the Congress, they could not Marshall the necessary support for the votes required to repeal the law. The coat benefits (political support), in this case, outweigh the cost (losing political support). Therefore, they are not likely to support the repeal.

The fact that if the Affordable Care Act is repealed will cause many people to lose insurance is troubling. The United States will have up to 21 million uninsured individuals (Abelson, Goodnough & Pear, 2019). In addition to, Abelson, Goodnough, and Pear state, “without the A.C.A, the cost of care for people who cannot pay for it could increase as much as $50.2 billion…Hospitals and other medical providers will incur losses” (2019). Therefore, one of the Affordable Care Act’s goals was to promote primary care and preventative care, which will reduce visits to the emergency department or reduce worsening chronic illnesses. According to Araujo’s Average Health Care Costs and Ways to Save, she states “health care spending is projected to grow at an average rate of 5.5% per year between 2018 and 2027” (2019). If the Affordable Care Act is repealed and the uninsured visit the emergency department without paying their bill, it will lead to money loss in health care. “Once a patient declared bankruptcy [for medical bills], the hospital had to declare a loss on any unpaid treatment” (Amadeo, 2019). Furthermore, in Amadeo’s article, she concludes, “Obamacare reduced the number of bankruptcy filings” (2019), which benefits the economy and Americans.

General notes/comments The Bill seeks to provide comprehensive attention to the care of persons with autism spectrum disorder. Still, there is a concern that the deadline for reauthorizing the Bill is 30th September 2019 and yet legislative debate is still ongoing to imply that the Bill may not be reauthorized by 30th September.

There is a need for the legislation to be reauthorized in its current form as the Autism CARES Act of 2019. That is because it seeks to increase the federal investment across the government departments concerned with addressing the increasing needs of persons with autism spectrum disorder. Should the Bill fail to be reauthorized by the 30th September 2019 deadline, then efforts to address autism spectrum disorders are likely to hindered and past gains reversed. In fact, failing to reauthorized the Bill would deny the Interagency Autism Coordinating Committee the authority to coordinate federal efforts targeted at autism. This would increase the possibility of efforts being duplicated and resources wasted. Besides that, autism is a present concern since 1.67% of children are suffering from the disorder, a significant population and yet management efforts are hindered by a dearth of information that would be best addressed through research. Firstly, research efforts would be coordinated through supporting research funded by the National Institute of Health. Finally, supporting training efforts to address the reported shortages of personnel who address screening, diagnosis and treatment efforts (Autism Society, 2019). Although the Bill seeks to improve autism management, there is a legitimate concern that it would increase federal spending on health and the federal deficit. This objection is in reference to Section 3 Part e of the Bill that amends appropriations authorization to increase funding for federal programs. This concern can be addressed through making two amendments to the Bill. The first amendment is to eliminate the part that seeks to increasing federal funding for autism management (repeal authorizations of appropriations) so that the federal deficit concerns are eliminated (Saba & McCormick, 2015). The second amendment is to mobilize funding from non-governmental and private donors who support a similar heath agenda. This would ensure that although federal funding is not used, the autism management activities identified by the Bill are not discontinued and they continue to receive funding (Feldstein, 2006). Overall, the Autism CARES Act of 2019 has much value, and could be improved to receive greater support if federal funding is reduced and alternative non-government funding is sourced. Credit Hours

Students examine the importance of getting healthcare issues on federal and state agenda, investigate governmental response through legislation and regulation, and identify where they can advocate for positive outcomes in program/policy design, implementation, and evaluation. Global health issues are examined for their relevance and impact on the development of the nurse advocate. Through discussions, reflection, case studies, and the application of professional communications techniques as political tactics to influence policy outcomes, students demonstrate integration of policy decision-making into professional nurse practice for the benefit of individuals and populations NURS 6050 Policy and Advocacy for Improving Population Health.

Primary and Secondary Sources

Review the following information prior to selecting resources for assignments.

Primary: A primary source is an original document that is the first account of what happened. A research report is primary, and you can tell because it includes materials and methods demonstrating how the research was done. Some creative work is also primary, such as poetry, novels, and interviews of people who experienced something firsthand. In nursing, which is an evidence-based discipline, we strive to use primary research that is published in scholarly, peer-reviewed journals.

Scholarly, peer-reviewed journal: Scholarly journals publish papers by professional authors and experts in the field using a peer-review process to review the work and assure quality before publishing. The focus of a scholarly journal is to provide accurate information for scholars and other researchers. The focus is on content rather than advertising, a direct contrast to popular media. Scholarly journals publish both primary and secondary papers, the former usually noted as original research and the latter as reviews and commentaries. Secondary: A secondary source is one step removed from the original source. Incomplete Grade Policy

Per University policy, Incomplete grades can be granted only to students who have already met the minimum criteria for active weekly participation in a course (including weekly postings in online courses) and have completed at least 80% of other coursework. Incompletes can be awarded when, because of extenuating circumstances, a student has not met additional course requirements, including but not limited to written assignments, group projects, and research papers, as applicable. Students who are eligible for an Incomplete must contact the Course Faculty to request the grade as soon as possible. Students who do not meet the criteria listed above will not be allowed to earn an Incomplete. If the Incomplete is approved, the Faculty Member will work with the student to outline the due date(s) for remaining work. Under no circumstances will the new due dates extend beyond 50 days from the last day of the term. Faculty will then have 10 days to assess the work and post the permanent grade before the University-allotted Incomplete time limit of 60 days expires. All Incomplete grades not resolved within the time allotted will convert to permanent grades of F NURS 6050 Policy and Advocacy for Improving Population Health.

  3. All e-mail correspondence must contain in the subject line “NURS 6050-XX-NAME” (XX is the section number) followed by a brief description of the subject. This subject line convention ensures that your e-mail will be easily identified and responded to in a timely manner. It is required that the e-mail contain a signature that matches the official name used in the course.

Late Assignment Policy

Students are expected to submit assignments by the due dates noted in the course. In extenuating circumstances, such as illness, the student must contact the Instructor as soon as possible to discuss the situation. In those circumstances, faculty will determine the appropriate course of action for the student. Depending on the situation, these actions may include recommendations to drop the course (if within the University drop/withdrawal period), acceptance of some or all of the overdue assignments with or without penalties, or failure to accept assignments.

Assignments submitted late without prior agreement of the Instructor, outside of an emergency absence, or in violation of agreements for late submission, will receive a grade reduction for the assignment amounting up to 20%. After 5 days, the assignment will not be graded. Keeping Your Coursework

Regardless of political affiliation, every citizen has a stake in healthcare policy decisions. Hence, it is little wonder why healthcare items become such high-profile components of presidential agendas. It is also little wonder why they become such hotly debated agenda items.

Consider a topic that rises to the presidential level. How did each of the presidents (Trump, Obama, and Bush) handle the problem? What would you do differently?

It may seem to you that healthcare has been a national topic of debate among political leaders for as long as you can remember NURS 6050 Policy and Advocacy for Improving Population Health.

Healthcare has been a policy item and a topic of debate not only in recent times but as far back as the administration of the second U.S. president, John Adams. In 1798, Adams signed legislation requiring that 20 cents per month of a sailor’s paycheck be set aside for covering their medical bills. This represented the first major piece of U.S. healthcare legislation, and the topic of healthcare has been woven into presidential agendas and political debate ever since.

As a healthcare professional, you may be called upon to provide expertise, guidance and/or opinions on healthcare matters as they are debated for inclusion into new policy. You may also be involved in planning new organizational policy and responses to changes in legislation. For all of these reasons you should be prepared to speak to national healthcare issues making the news.

In this Assignment, you will analyze recent presidential healthcare agendas. You also will prepare a fact sheet to communicate the importance of a healthcare issue and the impact on this issue of recent or proposed policy.

To Prepare:

  • Review the agenda priorities of the current/sitting U.S. president and the two previous presidential administrations.
  • Select an issue related to healthcare that was addressed by each of the last three U.S. presidential administrations.
  • Reflect on the focus of their respective agendas, including the allocation of financial resources for addressing the healthcare issue you selected.
  • Consider how you would communicate the importance of a healthcare issue to a legislator/policymaker or a member of their staff for inclusion on an agenda.

The Assignment: (1- to 2-page Comparison Grid, 1-Page Analysis, and 1-page narrative) with a title page, an introduction, purpose statement, and a conclusion. This is an APA paper.

Discussion: Politics and the Patient Protection and Affordable Care Act

Regardless of political affiliation, individuals often grow concerned when considering perceived competing interests of government and their impact on topics of interest to them. The realm of healthcare is no different. Some people feel that local, state, and federal policies and legislation can be either helped or hindered by interests other than the benefit to society.

Consider for example that the number one job of a legislator is to be reelected. Cost can be measured in votes as well as dollars. As a nurse, how often have you thought to yourself, If I had anything to do about it, things would work a little differently? Increasingly, nurses are beginning to realize that they do, in fact, have a role and a voice.

Many nurses encounter daily experiences that motivate them to take on an advocacy role in hopes of impacting policies, laws, or regulations that impact healthcare issues of interest. Discussion: Professional Nursing and State-Level Regulations

Boards of Nursing (BONs) exist in all 50 states, the District of Columbia, American Samoa, Guam, the Northern Mariana Islands, and the Virgin Islands. Similar entities may also exist for different regions. The mission of BONs is the protection of the public through the regulation of nursing practice. BONs put into practice state/region regulations for nurses that, among other things, lay out the requirements for licensure and define the scope of nursing practice in that state/region.

It can be a valuable exercise to compare regulations among various state/regional boards of nursing. Doing so can help share insights that could be useful should there be future changes in a state/region. In addition, nurses may find the need to be licensed in multiple states or regions.

Word cloud generators have become popular tools for meetings and team-building events. Groups or teams are asked to use these applications to input words they feel best describe their team or their role. Assignment: Advocating for the Nursing Role in Program Design and Implementation

As their names imply, the honeyguide bird and the honey badger both share an affinity for honey. Honeyguide birds specialize in finding beehives but struggle to access the honey within. Honey badgers are well-equipped to raid beehives but cannot always find them. However, these two honey-loving species have learned to collaborate on an effective means to meet their objectives. The honeyguide bird guides honey badgers to newly discovered hives. Once the honey badger has ransacked the hive, the honey guide bird safely enters to enjoy the leftover honey.

Much like honeyguide birds and honey badgers, nurses and health professionals from other specialty areas can—and should—collaborate to design effective programs. Nurses bring specialties to the table that make them natural partners to professionals with different specialties. In the Module 4 Discussion, you considered how professional nurses can become involved in policy-making. A critical component of any policy design is evaluation of the results. How comfortable are you with the thought of becoming involved with such matters?

Some nurses may be hesitant to get involved with policy evaluation. The preference may be to focus on the care and well-being of their patients; some nurses may feel ill-equipped to enter the realm of policy and political activities. However, as you have examined previously, who better to advocate for patients and effective programs and polices than nurses? Already patient advocates in interactions with doctors and leadership, why not with government and regulatory agencies?

In this Discussion, you will reflect on the role of professional nurses in policy evaluation NURS 6050 Policy and Advocacy for Improving Population Health.

Program/policy evaluation is a valuable tool that can help strengthen the quality of programs/policies and improve outcomes for the populations they serve. Program/policy evaluation answers basic questions about program/policy effectiveness. It involves collecting and analyzing information about program/policy activities, characteristics, and outcomes. This information can be used to ultimately improve program services or policy initiatives.

Nurses can play a very important role assessing program/policy evaluation for the same reasons that they can be so important to program/policy design. Nurses bring expertise and patient advocacy that can add significant insight and impact. In this Assignment, you will practice applying this expertise and insight by selecting an existing healthcare program or policy evaluation and reflecting on the criteria used to measure the effectiveness of the program/policy.

Throughout the United States the practice of a nurse practitioner varies from states to state. These laws and regulations can be defined as full practice, reduced practice or restricted practice (American Nurse Practitioner, 2018). I am from New Jersey and I am a current resident of Colorado, therefore, I will elaborate on the difference of regulations for nurse practitioners between the two states.
In Colorado nurse practitioners have full practice. Full practice means all nurse practitioners can “evaluate patients; diagnose, order and interpret diagnostic tests; and initiate and manage treatments, including prescribing medications and controlled substances” (American Nurse Practitioner, n.d.-a). Nurse practitioners have autonomous practice, can order physical therapy referrals, sign disabled placard cards and sign POLST forms and cannot sign death certificates (Barton Associates, 2019). In Colorado, a nurse practitioner can prescribe after a thousand hours of practice with a physician or another nurse practitioner (Scope of Practice, n.d.-a). Licensure requirements are an RN license, graduate degree and national certification (American Nurse Practitioner, n.d.-a).
In new Jersey, nurse practitioners have reduced practice. Reduced practice states “reduce the ability of NPs to engage in at least one element of NP practice” (American Nurse Practitioner, 2018), elements defined as “ordering of certain diagnostic tests, the provision of emergency medical services, the dispensing of certain medications and other treatments, and medical referrals” (Louie, n.d.). The nurse practitioner must collaborate and have a written protocol with a physician when prescribing medications. In addition to, the nurse practitioner “must consult the supervising physician when prescribing controlled substances” (Scope of Practice, n.d.-b). Furthermore, nurse practitioners must have “at least six extra hours of pharmacological education to prescribe medications and devices” (Nurse Practitioner Schools, n.d.). Nurse practitioners do not have autonomous practice, can order physical therapy referrals, can sign death certificates under specific situations, can sign disabled placard cards, and sign POLST forms. (Barton Associate, 2019). Licensure requirements are an RN license, graduate from an NP program and a national certification (American Nurse Practitioner, n.d.-b).
In both states, nurse practitioners are recognized as primary care providers.
