Introduction

Prior to starting this course, I was nervous about taking the pharmacology class due to the fact I have not had any experience or knowledge of the material learned. I believe the aspect of this course that was most beneficial to me would have to be the various assignments we did throughout the 5 weeks. When a professor assigns me an assignment, I can actually apply my knowledge instead of memorizing and forgetting the material. For example, you assigned quizzes, discussions, case studies, and reflections, which allowed me to gain a lot of knowledge from reading and researching about the particular subject. This class was not solely based on quizzes and tests like a lot of my other prerequisite courses, which allowed me to step out of my comfort zone. I feel confident in the knowledge I have gained in this course, and I now have a better understanding of pharmacology. To begin, I would like to mention the top 3 things I learned in this pharmacology course are, Pharmacodynamics and Pharmacokinetics , Decision-Making Related to the Use of Drugs in Dental Hygiene Care.

The first key thing I am taking from this course is understanding the Pharmacodynamics and Pharmacokinetics of drugs. Pharmacology encompasses two aspects of drug metabolism – pharmacokinetics and pharmacodynamics. While pharmacokinetics deals with drug absorption, distribution, metabolism, and excretion, pharmacodynamics deals with drug efficacy, safety, potency, and drug interactions. According to Haveles, the effects of a drug can be affected by a client’s general condition, age, and drug history or by the presence of disorders. The pharmacodynamic effect of a drug can be affected by a client’s general condition, age, and drug history or by the presence of disorders. (Haveles, 2020). Each of these may have an effect on binding at the receptor site or on the presence of necessary components that facilitate drug binding. In some cases, these effects can be toxic or lethal. Therefore, in terms of prescribing, pharmacokinetic principles suggest that the dosage may need to be adjusted to prevent undesired effects and/or adverse reactions.

The second key thing I would like to touch on is the importance of medication used in dentistry. I believe this topic is important to know is because depending on the patient’s condition, a dentist may prescribe a number of different medications, depending on your condition. Common medicaition used in Dentistry aree antibiotics and antiseptics which are employed to treat the ailments such as plaque and gingivitis, and anti-fungal medicines which are used to treat oral thrush. These medicines target the gums and the dental roots, and are available as oral pills, mouth rinses and gargles. They are also utilized to treat breath odor. Bacteriostatic as well as bactericidal antibiotics include penicillins, cephalosporins, tetracyclines, aminoglycosides etc. classes of drugs. The choice of antibiotics is driven by several factors including type of infection, age of the patient, compliance, medical history, concomitant medications, and bacterial resistance. It is also important to know other medications as well. For example, Hypertension is the most common Cardiovascular disease in America. According to the CDC, nearly half of adults in the United States (108 million, or 45%) have hypertension. It is important for the hygienist to identify it as well as

Decision-Making Related to the Use of Drugs in Dental Hygiene Care.

Lastly, a key topic that greatly stood out to me and I will be taking from this course is Decision-Making Related to the Use of Drugs in Dental Hygiene Care. I believe this is important because The medical history intake opens the door to effective patient-provider communication, which facilitates the evaluation of possible drug interactions, medication adherence, and possible implications for planned dental procedures. For example, a patient with a history of liver damage should avoid the consumption of pain medication of acetaminophen due to its toxic hepatic effects. Nevertheless, I believe this is important is because upon this decision-making skills and deductive reasoning needed to determine the appropriate drug therapies and recommendations.

Conclusion

Overall, looking in hindsight, I feel that advanced pharmacology did enhance my awareness and knowledge of pharmacology. I feel that it pulled things together that I did not necessarily realize that I did not completely understand. It increased my knowledge and promoted critical thought. It was also beneficial to understand the differences between states on practice authority for nurse practitioners. This was something that I was slightly confused about upon entering the course, but I was enlightened to the variances and the resources to ensure accurate information. Having a strong background and understanding in pharmacology is important, no matter what level of authority a nurse practitioner has- especially because the practice authority changes within states regularly. Because some states allow the nurse practitioner to prescribe all drugs, including controlled drugs, it is important to understand the pharmacokinetics and pharmacodynamics of each drug (Vacek & Vuckovic, 2019). Although there are new drugs all the time, it is good to have a strong foundation and know what resources are the best and available to keep up with new information regarding pharmacology. It is also important to stay current with continuing education, as a nurse practitioner. Staying competent in prescribing is vital, for both enhanced awareness in new drugs, clinical trials and prescribing laws

Case Study: A 65 year old obese female presents to the ED via ambulance with a compound fracture of the Left ankle. VS: BP= 175/92, HR= 110, RR=26, O2 sats= 95% on RA. Pt is diaphoretic and hollering in pain. Pulse +1 at site of injury. Unable to get PMH.  2mg Morphine given in the field.  Total of 1.5mg of Dilaudid given in ED. Routine lab drawn which indicates the patient has liver disease and a blood glucose of 365.  All other lab WNL.  X-ray tech is present to take film.  While adjusting left foot, patient experiences a level of conscious change. Patient becomes lethargic.  Arousable to stimulation but returns to previous state almost immediately.  VS: BP= 90/50, HR= 82.  Patient is apneic with sats 87% on RA.  When family arrived you learn that the patient takes Gabapentin daily for neuropathy.  The family thinks it is 300mg BID but can not remember for sure. Disease can effect the pharmacokinetics of drugs.  The liver disease noted in this case study can change the kinetics of drugs the liver has biotransformed (Rodighiero, 1999).  The metabolism and excretion of drugs can be delayed.  With these delays you can see a compounded and extended effect on the patient.  When administering medication, especially narcotics, one should be mindful of the presence of disease and how the patient will be affected.  According to one article, the author stated “the clearance of these drugs is reduced in patients with liver failure; thus, the initial dose may need to be lower, the interval between the doses may need to be increased, and such patients will need to be assessed on a regular basis” (Swetz et al., 2010, p. 959).

In this case, where pharmacodynamics are concerned, one must remember that all drug responses are not created equal.  One patient’s pain may respond significantly to 1 mg of morphine while another patient may not have the same effect until 2 mg of morphine have been given.  With this scenario, age and pathophysiological changes can contribute to increased effects and slow metabolism and excretion.  According to an article in The American Geriatrics Society, mixing opioids and gabapentin can produce overdose in patients indicating that substantial harm can occur (American Geriatrics Society 2019 Beers Criteria Update Expert Panel, 2019).

In developing a plan of care for this patient, education is key using a multidisciplinary approach.  If available and possible, having a family member present for teaching would be beneficial.  Providing information and teaching on each diagnosis will be beneficial and key to a successful management plan.  Having a pharmacist on the team that can evaluate current medications and provide necessary education can help prevent drug interactions that can cause adverse reactions.  I would have case management involved so they could identify areas of need that might affect good quality care and/or the ability to access resources needed.  Once the big picture is reviewed, there could be a need for referrals to other doctors, facilities, or disciplines.

With these patients that have liver failure it is important to get some specific labs that will give the clinicians an idea of the extent of the liver failure. This is an important addition to the care plan. These liver function and bilirubin levels will tell us a lot about the disease. Patients with severe liver disease, are on medications that lower the Bili levels. One example is taking Colace three times a day.This is given to bind some of the excess bilirubin in their stool. This causes frequent bowel movements. Patients complain about this effect because it disrupts their activities of daily living. So many patients omit a dose or omit all of the doses. This can increase  their Bili levels and their chances of hepatic encephalopathy. They can become unconscious. We need to know what has caused them to be unconscious.  When a patient is unconscious or is sub optimally breathing they can become acidotic. When these patients have frequent bowel movements, this puts them at risk for metabolic alkalosis. Whether a patient is in an acidotic or alkaline state, the medications they take can be greatly effected . They could overdose or underdose depending on their PH and coupled with the metabolism deficiency they have from hepatic failure.  Also when a patient has liver failure they have decreased albumin (M.Z.,n.d.) This can cause low blood pressure, increasing the likelihood of a patient passing out and becoming unconscious. When patients with liver failure have less circulating albumin this effects the medications that they may take that are protein bound medications (Rosenthal, &Burchum, 2021).This scenario can become even more dire if the patient is much older than 65 when liver clearance and renal function have a tendency to decline. Rosenthal states that the to better understand and properly prescribe medication to our patients one must understand the pharmacokinetics, or drugs movement throughout the body, as well as pharmacodynamics, or the drugs physiological or biological effect on the body. (Rosenthal, 2021) Both pharmacokinetics and pharmacodynamics are dependents upon the patient’s comorbidities as well as other pharmacological treatment, as many drugs could have a synergistic or antagonistic affect.

There is a growing concern of opioid use and subsequent tolerance this patient population can experience. “The term pharmacodynamic tolerance refers to the familiar type of tolerance associated with long-term administration of drugs such as morphine and heroin.” (Rosenthal, 2021) This result is an adaptation to the receptors for this type of medication as a result of long term or chronic exposure to the opioid narcotic. This forces the provider to administer more frequent and often higher dose levels to attain the desired effect.

This brings to mind one of many examples that I have encountered throughout my career. This particular patient was a 42-year-old white male with a history of uncontrolled hypertension, polysubstance abuse and bipolar disorder. This patient received coronary artery bypass surgery and was extubated post-operative day zero following an uneventful surgery. NURS 6521 Pharmacokinetics and Pharmacodynamics Discussion

This patient did require higher than normal levels of sedation while conducting the weaning process from the ventilator. Following the patient’s removal from the ventilator, pain management remained the focal point and largest obstacle for this patient. In order to affectively manage this patient level of comfort a continuous of morphine was ordered to achieve a baseline level of comfort. Dilaudid pushes where ordered at a frequency of every hour for breakthrough pain as well as Ativan pushes to assist with rising anxiety levels.

As an Intensive Care Unit (ICU) nurse of 10 years, I have taken care of a variety of different people with a broad spectrum of illnesses and problems. Pharmacokinetics, or how the body processes a drug, must be carefully considered for the best treatment options (Rosenthal & Burchum, 2018). A typical patient diagnosis that can be particularly difficult to manage medically is drug and alcohol abusers. Their drug tolerances and organ impairment related to damage from abuse can make it especially challenging to treat their conditions.

I vividly remember taking care of a patient in their early 30s who was admitted to the medical intensive care unit that I worked in for alcohol withdrawal. He had no past medical history except for alcohol and benzodiazepine abuse. He was found by a family member in alcohol withdrawal and was immediately brought to the hospital. The clinical institute withdrawal assessment for alcohol scale (CIWA), was used to determine the severity of withdrawal. This scale views different symptoms of withdrawal such as nausea/vomiting, tremors, sweats, anxiety, agitation, visual and auditory disturbances, headache, and orientation (MERCK Manual Professional Version, 2020). His CIWA score was extremely high, which showed severe withdrawal.

Typical treatment for alcohol withdrawal includes the administration of benzodiazepines in order to get through the detox process. This drug helps lessen the withdrawal symptoms. When giving this patient a standard dose of benzodiazepines such as Ativan or Valium, there was no decrease in clinical symptoms. This was because of his history of benzodiazepine abuse and increased tolerance. Increased drug administration and frequency were ordered until the patient’s withdrawal symptoms were under control. However, the patient also had liver damage from alcoholic cirrhosis. Benzodiazepines are drugs that are metabolized by the liver (Brett & Murnion, 2015).  Since the liver was not functioning correctly, the medications administered to control alcohol withdrawal were not processed appropriately by the liver, causing a buildup of the drugs (Louvet & Mathurin, 2015). The patient became unresponsive, and an advanced airway was placed for airway protection.

I feel that this patient’s care could have been handled differently. Due to this patient’s history, a different plan of treatment should have been initiated. I feel that by following the standard protocol for alcohol withdrawal treatment, the outcome of this patient getting intubated was inevitable. However, the patient will remain unresponsive until his body cleared all of the benzodiazepine drugs. This could be a lengthy process and delay his care further. I would have suggested intubation and mechanical ventilation from admission, and to use sedation safe for the liver. By doing this, the patient would be able to overcome his detox safely and have his breathing tube removed sooner. I have learned a lot from my experience with pediatric patient populations. First, the effective administration of drugs is tremendously challenging. For example, the gastric PH of a pediatric patient changes rapidly from birth through to childhood, which affects the efficiency of orally administered drugs. In review of literature one potential restraint that providers have when it comes to the pediatric patients is considering the likelihood that oral medications may be rejected due to the taste and texture that they are available in. It is essential for providers to understand the developmental pathway that occurs in terms of pediatric patients. According to Eidelman & Abdel-Rahman (2016), “the capacity to discriminate sweet and umami is present in utero, followed by texture, temperature and piquancy at 1–2 years of age and finally bitter, salty and sour around the age of 2 years. Olfactory senses do not appear to fully mature until children are 5–7 years of age. Other adaptive behaviors, which typically manifest by 2–5 years of age, influence a child’s willingness to accept novel foods, and by extension, medicines” (Eidelman & Abdel-Rahman, 2016, p.70). It is essential that providers are aware of all of the available forms and flavors that medications come in to ensure that medication is not rejected by the pediatric patient to ensure that the medication is effective. This course provides students the opportunity to integrate what they have learned in the program in a practicum experience. Students are expected to integrate nursing knowledge, knowledge gained throughout the program, leadership, and advanced critical-thinking and problem-solving skills in the development of a comprehensive and professional capstone project change proposal.

Student Specific Objectives: Students are responsible for completing all experiences as mandated for program.

A pre-conference will occur prior to the start of the clinical practice experience. This meeting is intended for the student and preceptor to review course and student-specific learning objectives: the roles, responsibilities, and expectations of student and preceptor during this practicum experience. All faculty and preceptor contact information will be given to each party per the student. Any course or program information requested by the preceptor will be provided by the student. Faculty will review submitted document.

A mid-conference/evaluation will occur at the mid-point of the course.
