Advanced nursing practitioners should have the opportunity to practice their skills to the optimum level without any limitations since they have undergone proper training and education (Spacey et al, 2021). Nevertheless, there are barriers that hinder advanced nursing practitioners from fully practicing their acquired skills which call for the need to drive new change so that they can have the opportunity to practice without set restrictions. This essay intends to document the specific barriers as well as interventions that can be put in place to eradicate those barriers.

The fact that various states in the United States have specific policies concerning state practice as well as licensing, causes barriers to advanced nursing practitioners (Markowitz & Adams, 2022). This is because many states have relegated advanced nursing practitioners by restricting them from fully practicing their nursing skills. For example, the full training and advanced education nurses go through equip them with skills to competently diagnose, assess patients, provide medication as well as effectively interpret diagnostic lab tests. This is because they force nurses to be under physicians and they are disregarded as the primary care providers meaning that they are not directly paid for their services and they receive lower reimbursement rates.

Concerning state practice as well as licensing, organizations such as the American Nurse Association should step in and allow advanced nursing practitioners to practice to their full extent considering that they are unaware of the skills nurses possess as primary care providers (Bates & Martin-Misener, 2021). For example, in my state Florida, local organizations like Florida Health Care Association (FHCA) can collaborate with organizations such as American Nurse Association to drive policy changes that allow the free practice of nurses in accordance with the set best practices.

Also with regards to physicians being considered more skilled, as long as the nurses have undergone the appropriate training and education, the healthcare sector should allow nurses to practice independently without the supervision of the physicians considering that in my state Florida, nurses are restricted from assessing patients because it is considered the role of the physicians. Lastly, variances in reimbursement rates should be eliminated in order to promote advanced nursing practitioners’ freedom of practice as they are the primary care providers compared to physicians (Peterson, 2017).

It is worth noting that existing barriers such as state licensure/scope of practice, physicians’ superiority/supervision, and varying reimbursement rates hinder advanced nurse practitioners from freely practicing to their full extent. Therefore, all these factors should be eliminated to ensure that they can practice to their full potential as primary caregivers.

As the medical field continues to advance each day so does the role of a nurse practitioner (NP). Although, faced with countless barriers they have become a health partner of choice that serve a diverse population across many healthcare settings. A nurse practitioner is considered an advanced-registered nurse qualified to provide a wide variety of health care services such as diagnosing diseases, writing prescriptions, constructing a treatment plan as well as executing bone marrow biopsy and lumbar punctures (Medline Plus, 2020). Furthermore, NPs also have the leisure of working in different specialties for example, Women’s health, Pediatric or Neonatology to name a few (Medline Plus, 2020). For years, demand has been high for the nursing profession, but it wasn’t until 2020 during the COVID-19 pandemic when we saw a drastic increase in nursing shortage as it became one of the healthcare system frontline workers. A nurse practitioner providing skills like those of a doctor or even attaining equal pay when doing so was insupportable. Sadly, the perspective of physicians providing better patient care compared to a nurse practitioner is dishonest and misleading. Nurses are patient centered, empathetic and knowledgeable. Therefore, when continuing our education to better serve our patients, it should be recognized and respected among our peers. The second barrier I believe a nurse practitioner encounter is the lack of power they hold when the patients they treat is admitted to an acute care facility (The Online Journal of Issues in Nursing, 2021).

Once a patient is admitted the nurse practitioner no longer can continue care which can cause major issues. The lack of privilege can result in a patient decline and an immense risk of complications (The Online Journal of Issues in Nursing, 2021). Lastly, the third and final barrier is minimal comprehension other healthcare workers have when it comes to the scope of practice of a nurse practitioner (NCBI, 2017). Due to this setback, finalizing patient care can be postponed and delayed. Ultimately, we all advocate for our patients however, if an NPs scope of practice is unbeknownst then orders may be delayed, and patient care will deteriorate (NCBI, 2017). Hence, to overcome these barriers the medical team should address the situation and formulate a positive outcome that will eventually benefit the care given to the patient. Non-pharmacological Treatment

C.B. has been diagnosed with diabetes and will need to take some steps to maintain a healthy blood glucose level. Healthy eating is one of the criteria. Because of the patient’s condition, a shift in diet toward whole foods, fruits, and vegetables is required. Sweetened beverages, white bread and rice, and dried fruit must be avoided since they contain high amounts of chemicals such as fructose corn syrup, which raise blood glucose to harmful levels. Furthermore, the diet contains a low intake of red meat and dried fruits, both of which induce insulin resistance in cells (Lester, 2019).

An increase in physical exercise is another strategy. Physical activity helps to maintain a healthy glucose level by enhancing insulin sensitivity and stimulating cellular glucose use. Insulin sensitivity in the skeletal muscles is reduced in type 2 diabetes patients, resulting in lower blood glucose uptake. Exercise improves this condition by translocating the glucose transporter 4 in skeletal cells from intracellular position to within extracellular access (Lester, 2019). This change results in improved kidney function, weight loss, and better blood glucose levels.

Pharmacological Treatment

Physical activity and diet play an important role in the management of diabetes mellitus. These measures, however, may be insufficient to provide instant relief. One of the pharmacological interventions C.B. may apply to attain quick results is the use of metformin. This drug, administered at 1,000 g/day, achieves weight loss by lowering gastrointestinal glucose uptake (Veni & Gupta, 2019). Metformin’s glucose control derives from its ability to alter cellular insulin sensitivity as well. Patients under this therapy experience about a 28% increase in glucose uptake in skeletal muscles and a reduction in blood glucose (Veni & Gupta, 2019). The drug also improves hepatic insulin production, which slows down gluconeogenesis.

Another pharmacological intervention is thiazolidinediones (TZDs). TZDs regulate blood glucose by improving hepatic and muscular sensitivity to insulin. This treatment activates intracellular nuclear receptors responsible for insulin-mediated glucose assimilation and its conversion into adipose tissue which lowers blood glucose (Carrasco-Sánchez et al., 2021). While TZDs fail to impact insulin secretion rates, the drug maintains low blood glucose by inhibiting the oxidation of fats, hence decreasing gluconeogenesis.

Those who use opioid medications for pain relief should minimize the duration of their use or consume a lot of fiber rich diets and take more water while on medications (Diaz, et. al, 2022).

Constipation: Risk Factors, Clinical Manifestations and Recommendations

The patient presents with bloating and passing of less than three bowel movements in one week, which are signs of constipation. Her stools are extremely hard, and she strains to pass them (Diaz, et. al, 2022). outline other signs and symptoms to be a feeling of the presence of a blockage in the rectum that prevents the bowel movements. Also, the patient may feel as though they have not completely emptied the rectum after passing the stool. Lastly, some patients experience pain when passing the hard stool due to a larger diameter than the rectum and its hardness.

I would consider the possibility of anemia in this patient since she has a strong risk factor that predisposes her to anemia. According to (Turner, Parsi, and Badireddy 2021), age beyond 65 years is a significant anemia risk factor. The patent in the case study is 74 years. Therefore, there is a high likelihood that the low hemoglobin levels in her blood due to advanced age caused her constipation.

Recommendations

The first initial nonpharmacological therapy is oral hydration using water. Discuss the contraceptive methods using the latest evidence-based guidelines that Karen would be medically eligible for. 

Evidence-based guidelines from the American College of Obstetricians and Gynecologists (ACOG) (2019) indicate that women with pre-existing medical conditions should avoid the use of hormonal contraceptives. Nevertheless, there are various contraceptive methods, apart from condoms, which Karen is medically eligible for. The first option is using a non-hormonal intrauterine device (IUD), also known as the coil. Secondly is using a diaphragm; this is a cup-like device inserted into the vaginal canal to prevent sperm from passing through the cervix to the womb. A third option is the calendar method, whereby the patient calculates their safe days and avoids sex during ovulation (ACOG, 2019).

Identify one method that you feel would be most beneficial to Karen and discuss why you selected it.

Out of all the contraception methods discussed herein, the most beneficial for Karen is the IUD. Firstly, the American Cancer Society (2020) asserts that IUD reduces the risk of cervical cancer; this is important for Karen because she has a family history of the condition. Secondly, the IUD is non-hormonal; this is beneficial for Karen, with DVT and cardiac disease history. Thirdly, according to the case “Karen does not desire pregnancy soon,”; IUDs are effective for as long as 5-10 years.

Are there any methods that you would not recommend for Karen? Why?

There are contraception methods that are not recommended for Karen. These include combined-injectables like depo provera, hormonal implants, and combined patch/vaginal rings containing hormones like estrogen (ACOG, 2019). This is because the use of such contraceptives in women with a history of DVT and cardiac illness may increase their risk of thrombus formation or venous thrombus.

  • Discussion Questions (DQ)

Initial responses to the DQ should address all components of the questions asked, including a minimum of one scholarly source, and be at least 250 words.
