Instructions for Individual Activity

Each student will choose one of the HealthyPeople 2020’s Environmental Health themes to investigate and explore how their community (chosen with Clinical Assignment #1: Community Assessment/Windshield Survey) is affected and impacted by this theme, as well as what is being done to address this environmental health theme to safeguard or improve societal and environmental health. Students should search relevant public/governmental agency websites and media/newspaper publications to help understand the situation from historical, present, and future oriented perspectives. The EPA’s My Environment website may also be helpful (/orders/www3.epa.gov/enviro/myenviro/). Students will then create a 2-page paper (approximately 500 words) that addresses the following:

· Briefly describe the selected environmental health theme.

· Discuss how the community is affected/impacted by this environmental health theme.

· Explore the selected environmental health situation from historical, present, and future-oriented perspectives.

· Discuss any strategies that are planned or have been implemented to address this environmental health theme to safeguard or improve societal and environmental health. Consider the barriers and facilitators for success of these strategies. Propose strategies if none are found.

· Within the paper, students should consider and address population health concepts such as “social justice” and “the greater good.”

· Student should utilize and appropriately cite relevant public/governmental agency websites and media/newspaper publications to substantiate their writing.

· Students will submit their APA formatted paper to Brightspace by the assigned due date.

Group activity

At the clinical site (or as directed by your clinical instructor), students will present and discuss their findings of the HealthyPeople 2020: Environmental Health Investigation paper developed in the individual activity portion of the assignment.

Together, group members will discuss their findings and the significance of the environmental health issues explored.

Immediately the first resource that comes to mind is UpToDate, a web-based application that is highly regarded by medical professionals for its reliability and communal forum. I was first exposed to UpToDate when I switched from working in the emergency department to the neurology department. The physicians and nurse practitioners of the team use it frequently and showed the benefits. UpToDate is a great resource as it provides evidence based information about many different medical diagnoses. One of the great attributes of the website is that it is as implied in the name, up to date on recent information and data. It is a reliable resource as it is peer reviewed and verified for the accuracy of the information provided. I have really grown to appreciate the section provided that includes a list of differential diagnosis that is provided on most medical condition pages. Nurse practitioners who support evidence-based nursing practice create a culture of inquiry and lay the foundation for the use of evidence to make informed clinical decisions (Majers & Warshawksy, 2020). Ideally I should incoporate more time reading new journals sent to me from my different memberships such as AAN and ANA as those will allow me to stay concurrent with new study and trial results. One barrier that we should be self aware of is the common aversion to change when new practices or suggested (Duncombe, 2017). Having an open mind to change from what is our routine will allow more room for growth. Participating in committees also allows a space for exposure to current and innovative practices that may occur at the facilities that we work at.

I have not yet been involved in the implementation of a new evidence-based practice intervention however, there is an intervention that would be useful in my workplace in distinguishing between viral and bacterial infectious etiologies while aiding in antibiotic stewardship, which is the use of procalcitonin testing. It has been estimated that 30%-85% of antibiotic prescriptions are unnecessary or inappropriate and even when indicated, often exceed recommended durations, leading to ever increasing antibiotic resistance.

Procalcitonin testing has been proven to be approximately 70% accurate in distinguishing bacterial from viral pathogen and when used in adjunct to algorithms and clinical judgement, has been shown to reduce unnecessary antibiotic use by up to 50% without increasing mortality or morbidity (Rhee and Mansour, 2022). A scenario where this may be useful is adding procalcitonin testing to ED order set that may be applied to presentations where there is high variability between viral vs bacterial etiologies, such as respiratory illnesses.

The implementation of evidence-based practice (EBP) follows a process that includes cultivating a spirit of inquiry, asking a clinical question, searching for evidence, appraising, and applying the evidence, and evaluating its application in clinical practice (Abu-Baker et al, 2021). The initiation of the use of procalcitonin would follow a process that includes modifying the implementation for the available resources such as initially using the testing in the ED to establish type of infection but not in the guidance of antibiotic use as part of a multiphase “rollout” process, establishing EHR order sets, and keeping patient centered goals in the forefront, which may include anticipating shorter hospital stays with reduced risk of hospital acquired infections. The most difficult stage is the implementation stage, which may be accomplished by time-intensive strategies, various models, staff education, computerized decision support, performance feedback, multiprofessional collaboration, media campaigns, and resources, which are often minimal (Tucker & Gallagher-Ford, 2019). Education and proving cost-effectiveness would be key in the implementation of procalcitonin testing in small hospitals such as the one at which I am employed, and thus, I would add a thorough cost analysis with projected savings both immediately and in the future.
