Analysis: Analyze the Christian worldview by addressing each of the following questions:

  2. What is troublesome or confusing about Christianity?
  3. How does Christianity influence a person’s thinking and      behavior?

Reflection: Reflect on your worldview by answering one of the following questions:

  1. If you are not a Christian, what similarities and      differences are there between your worldview and the Christian worldview?
  2. If you are a Christian, how specifically do you live      out the beliefs of the Christian worldview?

Conclusion: Synthesize the main points, pulling the ideas of the paper together.

Acute bronchitis

In the given scenario with Lou who is 35 and otherwise healthy, non-smoker with acute onset of tracheitis and a productive cough producing green sputum, it appears that it is acute bronchitis (Lefebvre, 2020). Acute bronchitis is a self-limiting respiratory infection that causes inflammation of the large airways distinguished by a cough (Lefebvre, 2020). The epithelial lining of the bronchi become thickened and inflamed (Lefebvre, 2020). As a result, it causes an interruption in airflow and bronchial hypersensitivity which leads to coughing, wheezing, and shortness of breath (Lefebvre, 2020). Sputum may or may not be present although it may appear as yellow or green from cellular debris (Lefebvre, 2020).

Clinical manifestations

The presentation of acute bronchitis typically exhibits generalized fatigue, fever, mild shortness of breath, and either a productive or nonproductive cough which can last from five days and lasting up to three to four weeks (Lefebvre, 2020; McCance & Huether, 2019). Further, a cough can be exacerbated by cold, dry or dusty air (Lefebvre, 2020; McCance & Huether, 2019). Despite the presentation of yellow or green sputum, it may not indicate bacterial infection as it may be present with viral bronchitis (Lefebvre, 2020; McCance & Huether, 2019).

Causes/risk factors

In a majority of cases, acute bronchitis is caused by respiratory viruses such as influenza A and B, parainfleunza, respiratory syncytial, corona, adeno and rhino (Lefebvre, 2020; McCance & Huether, 2019). In 6-15% of cases, bacteria are detected with haemophilus influenza and streptoccus pneumonia being the most prevalent (Lefebvre, 2020). In addition to these bacteria, less common or atypical bacteria include mycoplasma pneumoniae, bordetella pertussis and chlamydia pneumoniae have also been identified as causative agents (Lefebvre 2020).  Common risk factors may include smoking, asthma, allergies, older adults, childhood respiratory diseases and environmental exposures to pollutants (Lefebvre, 2020.

Factors influencing bronchitis

In consideration of the patient’s age and she does not have any chronic respiratory issues such as chronic obstructive pulmonary disease or emphysema which would put her at greater risk of pneumonia, her acute bronchitis should be resolve on its own (Grigoryan et al., 2017). Research has shown that up to 70% of acute bronchitis cases result in treatment with antibiotics (Grigoryan et al., 2017). In the 18–39-year-old age group, antibiotics were prescribed the most frequently in this population (Grigoryan et al., 2017). Antibiotics for otherwise healthy adults do provide any major benefit and may experience adverse effects of antibiotic treatment (Grigoryan et al., 2017; Lefebvre, 2020.

The patient is presenting with a persistent cough and is expectorating green sputum leading to the diagnosis of acute bronchitis. McCance & Huether (2019) define acute bronchitis as an inflammation or infection of the bronchi or airway caused by a virus. The patient is coughing as a result of a protective reflex from the vagus nerve. Coughing allows the individual to expectorate her secretions from the lower airways (McCance & Huether, 2019). The feeling of gagging and dry heaves is caused by the patient’s deep cough with the goal of expectorating the tenacious, thick sputum. Viral infections, allergens, or pollutants might trigger an inflammatory response in the bronchi resulting in acute bronchitis. When the bronchial wall is inflamed, mucosal thickening and epithelial-cell desquamation occur (Smith et al., 2020). The inflammation and infection on the bronchi create secretions causing irritation leading to a strong cough.

     According to this case, Lou most likely developed acute bronchitis as a result of a viral infection as she does not have a medical history or history of smoking. Common clinical manifestations of acute bronchitis include coughing, expectoration, chest pain, shortness of breath, and wheezing from baseline versus after 7 days of treatment (Nowicky & Murray, 2020). Pulmonary environmental toxins or allergens might play role in Lou’s condition, such as nitrous oxides, methane, arsenic, lead, cadmium, and air pollution. Two patient factors that heavily influence the diagnosis of acute bronchitis are age and behavior. Patients over the age of 65 and children younger than two years are at risk of acquiring infections (Nowicky & Murray, 2020). Common pathogens include influenza virus A and B, parainfluenza, and rhinovirus. Implementing infection precautions and getting routine vaccinations are preventable measures vulnerable populations can engage in.  Furthermore, behaviors like smoking can develop into chronic obstructive pulmonary disease (COPD).  Individuals with COPD are more likely to develop chronic bronchitis (Nowicky & Murray, 2020). All in all, Lou’s acute bronchitis can be managed with antibiotics and cough expectorants.

Competing Needs and their Impact on Policy

The needs of patients, workforce, and resources are competing needs that impact the development of policy tremendously. The agenda that the policy is advocating for should mitigate the competing needs. An example of this would be the shortage of nurses in the workplace. The continuous shortage of nurses is due to high turnover rates, lack of potential educators, and inequitable workforce distribution (Haddad et al., 2022).  If a policy adequately addresses staffing issues, then patient and workforce needs are met and costs are decreased.

If a policy does not adequately address staffing issues, then there could be numerous negative impacts. According to an article on RevCycleIntelligence.com, it has been shown that inefficient or ineffective healthcare workforce management can increase labor costs significantly (RCI, 2020). Not only does it affect labor costs, but it compromises the safety of both nurses and their patients as well. Inadequate staffing leads to work overload of available nurses, which leads to burnout. A study has that there is a significant relationship between decreased patient safety and increased levels of burnout (Garcia et al., 2019).

Competing Needs and their Impact on COVID-19

One competing need that has been impacted by COVID-19 is the decrease in essential resources healthcare workers need to perform their job duties, such as personal protective equipment, or PPE. The use of PPE has significantly increased since the rise of COVID-19. In fact, the World Health Organization (2020) stated that there is a severe disruption to the global supply of PPE. Implementing a policy regarding PPE can save facilities resources and financial costs. For example, the policy could address the proper donning and doffing, sanitation, and storage of PPE after each use for full optimization of supplies. In turn, costs are lowered, efficiency is increased, and the facility’s staff has the necessary resources to carry out their job duties.

As a nurse working in healthcare, it is vital to provide safe, quality care to my patients despite the limitations of resources. To provide this care, policy is developed to regulate care and protect nurses. Such policies may include controlling patient to nurse ratios and policies to protect patients’ confidential information. Policy, however; is influenced by completing resources.

Many competing needs affect the nurse shortage. The primary goal of a healthcare worker is to provide safe quality care to patients. Hospitals and other healthcare organizations also must work to operate at a sustainable cost. The balance of profitability and quality of care can be hard to maintain. Moreover, the healthcare company must balance the quality of the employee’s life (Akinleye et al., 2019).

Due to nurse shortages, healthcare workers must continue to work in high stress environments. These high stress environments are aggravated by high patient loads, complex patients, and high acuity of patients. This is combined with less time to manage tasks and a lack of resources to accomplish day-to-day tasks. The nursing shortage contributes to a healthcare organization having to allocate this high number of critically acute patients to a single nurse in efforts to increase profit. For this reason, policies are needed to ensure safe, quality care while saving money (Akinleye et al., 2019).

Nurse burnout can also contribute to nurse shortages. At my facility, many nurses left bedside after the strenuous COVID-19 season. Studies have shown that workers during the pandemic worked longer hours with more acutely sick patients (Lopez et al., 2021). For this reason, I would conclude that the pandemic was a competing factor in nurse shortages. My healthcare organization tried to develop policies to help nurses achieve adequate work-life balance. For example, the hospital established a policy that provided an opportunity for nurses to get discounted prices at the wellness center.

A factor that may also contribute to the stressor of the nurse shortage is the need for a facility to adopt technology. A healthcare facility might need nurses, however; leadership might choose to direct their financial resources toward maintaining health information technology versus recruitment or hiring needs (Kelly & Porr, 2018).

Several things can be done to address these completing needs. First, policy can be made to enhance educational resources for nurses. For example, if a nurse that typically works on a medical surgical floor is required to work in a critical care unit due to shortage of available critical care workers, this nurse needs to be educated throughout. If these nurses are provided better education safe quality care can be achieved. Moreover, as I mentioned before, the pandemic added another level to patient care. Education of the virus and its effects on the body system would be beneficial. According to a study, nurses do not always feel adequately trained to care for these individuals (Sperling, 2021).

Ethical dilemmas in nursing occur when a nurse is forced to choose from two or more options that tend to impact the welfare of a perceptive being. The available options which the nurse should choose from are mostly justifiable to be good. However, the person subjected to the situation must be sure of the most effective and efficient option to attain the desired results (Rainer et al., 2018). Choosing one position contains uncertainties that make an individual hesitate when subjected to a situation that requires careful selection.

The nursing field contains various situations that call for the medical providers to make critical decisions that significantly impact the course of action (Rainer et al., 2018). Therefore, nursing professionals must be trained on the same subjects and common skills in the learning institutions to handle ethical dilemmas in real-life situations (Haahr et al., 2020). Nonetheless, applying the acquired skills at the end of the training depends on the nursing practitioner’s knowledge. Furthermore, the clinical setting tends to influence the skills at the disposal of the nursing healthcare provider. While hospitals struggle to adopt procedures to promote smooth running within the health care setting, nurses face different challenging situations that need critical thinking. Ethical dilemmas are among such challenges since it depends on the understanding of every person and the expectations that need to be achieved (Haahr et al., 2020). Although nurses are concerned with the clients’ well-being, the unknown challenges need to be evaluated to solve the issue for desirable results.

A few months ago, I was in a very challenging situation at work. It was on a Tuesday when I arrived at the clinic. A few moments later, a male patient was brought into the emergency room with a head injury that caused a lot of bleeding. The injury occurred after the patient was involved in a road accident. The patient was bleeding heavily, especially from the left side of the head. Thus quick intervention was necessary to control the situation as the patient waited for specialized medical attention. Unfortunately, the patient had been brought to the clinic by well-wishers. Therefore, there was no one to consent to the treatment. Also, the patient was in a critical condition to give consent. At this point, I was in a dilemma on how to approach the situation. However, guided by the ethical principle of beneficence that requires health professionals to consider any procedures to do good for patients, I embarked on providing first aid to control the bleeding before organizing for the patient to be transferred to an emergency room. Later on, the patient’s family visited the clinic and accused me of treating their patient without informed consent. As a result, the family declined to pay the medical expenses. Since failure to obtain informed consent is a crime, I was required to meet the medical costs and discharge the patient before full recovery.

Introduction

An ethical dilemma is a challenge in deciding between two possible options which are not ethically accepted. In nursing field, it occurs when a nurse has to choose between two competing values and acknowledge the consequences (Faubion, 2022). Also, the ethical dilemma might be against the nurse’s code of ethics and personal values.

Scenario

Jane was my client during my community health experience. She was a 30-year-old university student with twenty weeks of pregnancy and a devoted Christian who firmly believed that God was capable of doing anything to his followers. Moreover, she had a history of heart disease precipitated by pregnancy. The physicians and clinicians were concerned that pregnancy could precipitate her condition resulting in a life-threatening emergency. They explained that her pregnancy stresses the heart and circulatory system by increasing the blood volume to almost 50% to sustain the fetus. In addition, as the heart rate would increase as result of overload worsening her condition (Mayo clinic, 2022). Also, the team demonstrated how labor and delivery could add to hearts workload. They told her that pushing during delivery would lead to abrupt blood flow and pressure changes, which was a threat to her life. After further consultation, the health team recommended abortion in the next three weeks. Jane was unwilling, stating she was raised in Christ, believing abortion is sin and God was in control. She wanted to know if I was in her position. I believe in abortion only on the medical ground, her condition included.

Ethical Dilemma

Usually, patients seek medical advice from healthcare providers. In this case, the medical intervention contradicts the patient’s religious beliefs and morals. Advising the patient on the best decision, which contradicts their morals and beliefs, creates an ethical dilemma.

Solution

According to the American Nurses Association Position statement on reproductive health, “healthcare clients have the right to privacy and make decisions about personal health care based on full information and without coercion. Also, nurses have the right to refuse to participate in a particular case on ethical grounds. However, if nurses’ life is in jeopardy, nurses are obligated to provide for the client’s safety and avoid abandonment (Haddad and Geiger, 2022).” Although I preferred abortion, I didn’t express my opinion and beliefs to the patient. Instead, I encouraged the patient to communicate with her significant others, including the husband, family members, spiritual advisors, and others who she felt they could confide in and seek advice. Also, I encouraged her to research the benefits of abortion to individuals with the same condition and even go further to find the experience of those who had life-saving abortions. It worked so well because she acknowledged her condition and the danger ahead. Additionally, I scheduled a follow-up visit with other healthcare teams to discuss the options available and make decisions within two weeks before the procedure was done. Unfortunately, we found none. Her heart condition was advanced, and only abortion was found effective and safe.
