1. Review the feedback you received from your instructor for Milestone 1, and use it to develop this milestone.
2. Create a proposal for your Design for Change Capstone Project. Open the recommended assignment specific Milestone 2 Design Proposal Outline (Links to an external site.)Links to an external site.. Use this to write your paper. You will include the information from Milestone 1, your practice issue and evidence summary worksheets, as you compose this proposal. Your plan is to convince your management team of a practice problem you have uncovered that is significant enough to change current practice.
3. The format for this proposal will be a paper following the Publication manual of APA 6 th edition.
4. The paper is to be four- to six-pages excluding the Title page and Reference page.
5. As you organize your information and evidence, include the following topics.
   1. Introduction: Write an introduction but do not use “Introduction” as a heading in accordance with the rules put forth in the Publication manual of the American Psychological Association(2010, p. 63). Introduce the reader to the plan with evidence-based problem identification and solution.

6. Citations and References must be included to support the information within each topic area. Refer to the APA manual, Chapter 7, for examples of proper reference format. In-text citations are to be noted for all information contained in your paper that is not your original idea or thought. Ask yourself, “How do I know this?” and then cite the source. Scholarly sources are expected, which means using peer-reviewed journals and credible websites.
7. Tables and Figures may be added as appropriate to the project. They should be embedded within the body of the paper (see your APA manual for how to format and cite). A: Sheila Meyer, a medical assistant in Dr. Ryan’s large cardiovascular practice, is taking the medical history of Edna Helm, an obese elderly woman with congestive heart disease. Edna states, “I’m always short of breath, and I perspire all the time. I guess I’m gaining weight, but the funny thing is that only my legs seem heavier. My heart is pounding when I lie down at night; it even seems to stop sometimes. I’ve even started to wear red nail polish to hide the funny blue color of my nails.

Edna gives you a copy of her medical history from an out-of-state physician. The medical history indicates that she has had the following conditions, tests, and procedures:

B: Victor Krenz is assisting Dr. Connors with the fifth cataract surgery for the day. The patient is Kathy Wall, a diabetic patient, whose condition has been stable enough for her to undergo a surgical procedure. Victor has performed a six-minute surgical scrub on his hands before each of the five procedures. Dr. Connors indicates that he is in a hurry to get back to his office for a heavy afternoon schedule of patients. After both Dr. Connors and Victor are scrubbed, gowned, and ready to begin the operation, Victor feels a slight prick on the tip of his gloved finger as he moves the sterile syringe and needle on the tray. Dr. Connors, who does not notice the accidental needlestick to Victor’s glove, states again that he is in a hurry to finish this procedure. Victor knows that it will delay the surgery if he has to change gloves. He also knows that his hands have had a surgical scrub five times that morning and that they are clean.

Provide detailed answers for each of the following questions. Your response should be one-half (1/2) page in length.

C: José Menendez is an elderly patient of Dr. Juárez, a board-certified urologist. José has a history of recurrent UTIs dating back more than 10 years. When he becomes symptomatic, he has been instructed to call Dr. Juárez’s office and schedule a urinalysis. Dr. Juárez’s receptionist has just received a call from Mr. Menendez. He says he knows he is supposed to come in for a urine test but that he just wants a prescription phoned in to his pharmacy instead. The receptionist asks Emilia, Dr. Juárez’s medical assistant, to take the call from Mr. Menendez.

Emilia listens as Mr. Menendez recounts that he is experiencing dysuria—painful, burning urination. She asks him to come in for a urinalysis, explaining that, as per standing orders, a clean-catch midstream specimen needs to be collected. Mr. Menendez repeats to Emilia that he does not want to come in to the office. “Why can’t you call in a prescription for Bactrim? That is what I took last time, and it helped.”

Provide detailed answers for each of the following questions. Your response should be one (1) page in length.

A: Marge Riley, an overweight 50-year-old woman with a history of abdominal pain, has been scheduled for a lower GI series on Monday morning. She states she has board meetings every Monday morning at which breakfast is served and that she will come in for her x-rays after the meeting is over. Marge indicates that she does not understand why she needs these procedures.

Provide detailed answers for each of the following questions. Your response should be at least 150 words in length.

· What, if anything, would you tell the patient regarding her need for these procedures?

Mr. Ivy, who is 70 years old, is weakened on the left side of his body from a cerebrovascular accident (CVA). He weighs 200 pounds and is reluctant to provide much help to Jenny when she has to transfer him from the wheelchair to the examination table.

The patient is an elderly woman, with multiple medical problems, who is possibly having a reaction to a medication that Dr. Waring prescribed two days ago for bronchitis. Her symptoms include nausea, upset stomach, dizziness, headache, rash on her chest, and extreme exhaustion. Theresa senses that the patient may be exhibiting some disorientation to time and place, because it is difficult to elicit consistent responses from her regarding her medications. The patient reports to Theresa that the newest medication she has been taking is Biaxin. The other medications she takes include Prinivil, Cardizem CD, Premarin, Prilosec, Robaxin, Zocor, Ambien, Prozac, Fosamax, Seldane, and aspirin. The patient does not know the dosage of any of these medications but is willing to “open up her bag of medicine” and read each prescription label to Theresa.

The clinic tends to hire medical assistants who are certified, experienced, and very capable of dealing with patients from different age groups, races, and cultures. However, Sarah is being considered for the position because, first of all, her father is a personal friend of Dr. Keeler and, second, qualified medical assistants are difficult to find because of the high demand.

After, I would like to meet with the grandmother alone in order to ask about the  teenagers parents and ask regarding any issues she would like to discuss with me without the presence of the grandkid, in many occasions grandparents like to speak to the health care providers in private without the presence of the minor. In this case the grandmother might be needing help from social work services regarding their housing situation. Assuming the grandmother takes great care of the patient and offers him all the necessary support, living in a high density public housing complex can be challenging for a 14 year old male, besides that,  not having both of his parents actively participating in the teenager life can  sometimes bring physical, social and emotional consequences.  Then I would like to ask questions regarding the patient’s family history and the medical history of both parents if known.  Health risk assessment is the core of health promotion and disease prevention regardless the patient’s age, in order to conduct a risk assessment is important to always assess for family history (Wu & Orlando, 2015).

Consequently I would ask for permission to speak to the adolescent in privacy, by doing this I build a sense of trust with the patient, allowing him to be more open regarding issues that he might not want to discuss in front of his grandmother, for example,  drug use,   the need for emotional support, etc.  It is always important to make sure the questions are clearly understood, medical and technical terminology is sometimes avoided in order to adapt to the patient’s language level (Ball, Dains, Flynn, Solomon & Stewart, 2019).

            Based on my assessment in this given case scenario, I will be utilizing the CRAFFT questionnaire,  this risk assessment tool is used as a screening tool for alcohol and substance abuse in adolescents, due to this patient’s living conditions and lack of parental support this patient may be at risk for emotional instability that can lead to drugs and substance abuse.

A small community hospital in the Midwest has used a homegrown information system for years. The system began in the early 1970s with a financial module. Over time, additional modules were added. A limited number of departments selected a commercial system and interfaces were used to integrate these into the overall functionality of the hospital information system. Except for physicians, most in-house clinical or care-related documentation is online. However, about 15% to 20% of this documentation is done by free text and is not effectively searchable. In addition, the screens, including the drop-down and default values, were built using terms selected by the in-house development team in consultation with clinical staff; thus there is no data dictionary or specific standard language. In the last few years, the hospital has purchased two outpatient clinics (obstetrics and mental health) and a number of local doctor practices. The clinics and doctors’ offices are now being converted to the hospital administrative systems. A few of the clinical applications that are tied directly to the administrative systems such as order entry and results reporting are also being installed.

A major change is being planned. A new chief information officer (CIO) was hired last year and she has appointed a chief medical information officer (CMIO) and a chief nursing information officer (CNIO). No other significant staff changes were made. With her team in place, one of the CIO’s first activities was to complete an inventory of all applications. Rather than continue to build, a decision was made to switch to a commercial vendor and the hospital selected a commercial system.

As a member of the clinical staff with informatics education, the CIO has requested that you develop a training and information presentation for the clinical staff that will:

1· Identify two or more issues with the existing system

2· Provide staff with appropriate “work-around” for using the existing system
