Discussion: Comprehensive Integrated Psychiatric Assessment

Many assessment principles are the same for children and adults; however, unlike with adults/older adults, where consent for participation in the assessment comes from the actual client, with children it is the parents or guardians who must make the decision for treatment. Issues of confidentiality, privacy, and consent must be addressed. When working with children, it is not only important to be able to connect with the pediatric patient, but also to be able to collaborate effectively with the caregivers, other family members, teachers, and school counselors/psychologists, all of whom will be able to provide important context and details to aid in your assessment and treatment plans.

Some children/adolescents may be more difficult to assess than adults, as they can be less psychologically minded. That is, they have less insights into themselves and their motivations than adults (although this is not universally true). The PMHNP must also take into consideration the child’s culture and environmental context. Additionally, with children/adolescents, there are lower rates of neurocognitive disorders superimposed on other clinical conditions, such as depression or anxiety, which create additional diagnostic challenges.

In this Discussion, you review and critique the techniques and methods of a mental health professional as the practitioner completes a comprehensive, integrated psychiatric assessment of an adolescent. You also identify rating scales and treatment options that are specifically appropriate for children/adolescents.

Operative Indications: this is a pleasant female who come in with soft tissue lesion in the left

Consequently, the procedure, risk, benefits, and alternative of excision of this lesion were discussed with the patient, and she understood and wished to proceed with the excision.

Operative Findings: Large soft tissue lesion measuring about 6 cm in greatest dimension

Operative procedure: The patient was brought into the operation room after informed consent was obtained. The patient then underwent sedation with a sterile prep and drape. We then anesthetized with 1% lidocaine with epinephrine and sodium bicarbonate, made a 4-cm incision, and used sharp dissection to dissect circumferentially around this soft tissue lesion, being careful to make sure that we were not entering any kind of hernia sac secondary to Spigelian hernia. As we continued to dissect circumferentially around it without sharp dissection, it was noted that it did go down to a muscle, but no evidence of hernia was identified. The lesion was excised in its entirety and electrocautery was used to control hemostasis. We then irrigated with saline solution and closed the subcutaneous tissue with 3-0 Vicryl interrupted and sutures. We then used 4-0 Vicryl sutures to close the skin in a subcuticular fashion. Benzoin and Steri-Strips were then applied. Blood loss was minimal. The patient tolerated the procedure well and remained in stable condition throughout the procedure.

variety of issues (ethical, professional, and legal) from both the personal and hospital perspectives

must be considered. SCENARIO

You are a nurse in the emergency room, working the Friday 7 p.m. to 7 a.m. shift, and your evening has

been filled with the usual mix of drunken belligerent teens, wailing babies, chronic obstructive

pulmonary disease (COPD) exacerbations, falls, fractures, and the routine, regular congestive heart

failure (CHF) patients. Your best friend is texting you from the concert that you had to miss tonight

because you were scheduled to work, and you respond to her between care of patients, jealous that she

is there and you are not. “What a jerk to torture me like this!” you think to yourself.

It is now 2 a.m., and the medics radio once again, notifying you of an incoming motor vehicle accident

victim, ETA of 5 minutes. You sigh and opt to use the restroom, rather than getting that much‐needed

cup of coffee, and prepare a room for your next patient. The medics roll in and begin to fill you in. The

patient is a 28‐year‐old male, a passenger on a bus that was involved in a crash, leaving the vehicle

overturned after rolling over an embankment. There were several fatalities among the bus passengers,

and “this victim has remained unconscious, though his vitals are currently” . . . and as you start to focus

on the patient, you take a second look. Can it be? It is! The lead singer, Jerod, from the band “Blue

Lizards,” who you have adored since you first heard his voice! The band had just left the concert that

you had missed last evening when the accident occurred. You quickly text your best friend . . . “Can you

believe?” and she responds with “Yeah, right. PROVE IT.” So you quickly snap a picture with your

smartphone, when alone with the patient, and send it to her. Can’t hurt, right? Celebrities are “public

property,” and that’s a part of their life, right? Just for good measure, you snap a few more pictures of

the unconscious singer in various stages of undress and then a shot of his home address, phone number,

and demographic information from his electronic health record. You sit your phone down on the

bedside table for a minute as you continue your assessment of the patient.

At 7:00 a.m., you drag your tired body home and straight to bed after a long but eventful night.

Theoretical Assumptions

  • Aging is a progressive process requiring from simple to increasingly complex adaptation;
  • Aging may be successful or unsuccessful, depending upon where a person is along the continuum of progression from simple to more complex adaptation and the extensive use of coping processes;
  • Successful aging is influenced by the aging person’s choices;
  • The self is not ageless (Tornstam, 1996); and
  • Aging people experience changes, which uniquely characterize their beliefs and perspectives as different from those of younger adults (Flood, 2006a).

Definition of Theory

The theory of successful aging describes the process by which individuals use various coping mechanisms to progress toward desirable adaptation to the collective physiologic and functional changes occurring over their lifetime, while maintaining a sense of spirituality, connectedness, and meaning and purpose in life.
