Literature has indicated COPD is the third leading cause of death, with an annual cost close to $50 billion across the country (Pruitt, 2018). While the literature indicate that 30-day readmission of COPD patient is preventable, it is unknown at University Hospital if proper discharge planning, follow-up care, and tele-health impact the reduction of 30-day readmission.
