NUR 520 The Psychopathology of Mental Health Patients Case Study

Description

Analyze and apply critical thinking skills in the pharmacokinetics and pharmacodynamics of psychopharmacological agents for patient treatment and health promotion while applying evidence-based research.

Beth is a 23-year-old Asian American graduate student. She is currently in treatment with a psychologist for anxiety. She claims that her cognitive therapy has helped her control her worry and anxiety, but she has noticed that her symptoms have worsened this year with all of the pressures of school. In addition to worry, her mind races at night when she tries to sleep, and she claims it may often take her two to three hours to finally fall asleep. She describes herself as a “type A” person who is rather “anal” about doing well. She worries nonstop about grades but also worries nonstop about her health, money, her parents’ health, and world affairs. She is very insightful about her condition and agrees the worry is excessive and unwanted. She admits that there is really no basis for the worry, as she has a full scholarship, she and her parents are in good health, and she currently has a 3.9 GPA. She claims that she has always been rather tense, but it didn’t get to the point that she sought treatment until her senior year in college. She was worried about getting accepted to grad school and “obsessed” over getting straight A’s. She sought the help of a psychologist at the university counseling service, was placed on Paxil, and began therapy. For the most part, her symptoms disappeared, but she remembers feeling tired during the day.

  • Summarize the clinical case.

This is the case of a 45-year-old African woman who began experiencing anxiety, inability to fall asleep, a general sense of daytime tiredness, anorexia, inappropriate guilt, sorrow, and feelings of worthlessness three months’ before her separation from her husband due to an extramarital affair. A lower extremity neuropathy of unknown origin was discovered in the past medical history, and no personal or familial psychiatric issues. Her social history disclosed that she consumes alcoholic beverages on occasion.

Create a list of the patient’s problems and prioritize them

Prioritization Issues Rationale
Her 25-year marriage ended two months ago after he had an extramarital affair. As a result, the patient will gain trust and realization in all aspects of life with the help of the psychiatrist. Learning to move forward and maintain a healthy lifestyle
Unknown cause of lower extremity neuropathy We could predict what would impact her health in daily life based on her medical history. Taking care of the issue will solve and mitigate further complications.
Over the last three months, she experienced increased anxiety, difficulty falling asleep, and daytime exhaustion. The impact of an unhealthy lifestyle can be reduced by identifying and modifying the causative factors.
An increase in sadness, improper guilt, a loss of appetite, and a reduction in energy Following the consultation and psychotherapy, the patient is now trying to recover and accept life’s realities.

Which pharmacological treatment would you prescribe? Include the rationale for this treatment

We cannot rule out or prescribe any therapeutic intervention or medicine to help the patient. If we proceed without the proper steps/consultation, the patient’s condition will worsen, or we will get the opposite if we expect positive outcomes. For instance, if a patient exhibits the same symptoms as a patient suffering from depression and the physician affirms it without conducting a test or examination, there is a possible medical mistake (Mullainathan & Obermeyer, 2022). Instead of ensuring a safe and good quality of care, we are endangering patients by disregarding hospital rights and procedures. As a result, taking the proper first step for a patient, including seeing a doctor or consultant before doing anything else, is critical to boosting healing.

Which non-pharmacological treatment would you prescribe? Include the rationale for this treatment

Making your mental health a priority is a huge step toward healing, especially if you’re in a bad mood. I’m going over each of your concerns one by one. So, here’s a list of things to do to live a healthy lifestyle:

  1. Get enough exercise: Work out for 30 minutes every day to improve your health and release feel-good endorphins, which can help some individuals handle or prevent anxiety and depression (Sharma, 2019).
  2. Getting enough sleep: Get at least 5 hours of sleep each night. Sleep deprivation causes your body to shut down and renders you ineffective (Fitness et al., 2020)
  3. Maintain a healthy diet: Maintaining a healthy diet will help people manage the ups and downs of their day.
  4. Meditation: According to Hopkins, people who participated in an eight-week insight meditation course improved their stress, anxiety, and pain symptoms.
  5. Know your boundaries: avoid undue stress as much as possible, such as having parties with people you don’t like. However, some stress may motivate you to enhance and overcome your difficulties. Setting work boundaries, such as not answering emails after a certain time, can help relieve anxiety, but if you don’t, you may be able to refine your skills in some cases, which is a good thing (Graves & Karabayeva, 2020).

Include an assessment of treatment’s appropriateness, cost, effectiveness, safety, and potential for patient adherence

Helen, for example, requires more advice and counseling before determining whether or not she requires some of the required medications. The patient will develop self-control and realize that they can reclaim their life during the assessment process. It’s more of a case-by-case situation. As discussed, patients’ compliance with appropriate treatments is not a problem; however, we as medical providers must first identify the issues and history to take steps to restore the patient’s life, particularly in this case. Helen, unlike her family, does not have a history of mental illness. As a result, we can use natural preventative measures or methods to help her gain self-identity and avoid developing depression (McGrat, 2019).

  • Discussion Questions (DQ)

Initial responses to the DQ should address all components of the questions asked, including a minimum of one scholarly source, and be at least 250 words. Successful responses are substantive (i.e., add something new to the discussion, engage others in the discussion, well-developed idea) and include at least one scholarly source. One or two-sentence responses, simple statements of agreement or “good post,” and responses that are off-topic will not count as substantive. Substantive responses should be at least 150 words. I encourage you to incorporate the readings from the week (as applicable) into your responses.

  • Weekly Participation

Your initial responses to the mandatory DQ do not count toward participation and are graded separately. In addition to the DQ responses, you must post at least one reply to peers (or me) on three separate days, for a total of three replies. Participation posts do not require a scholarly source/citation (unless you cite someone else’s work). Part of your weekly participation includes viewing the weekly announcement and attesting to watching it in the comments. These announcements are made to ensure you understand everything that is due during the week.

  • APA Format and Writing Quality

Familiarize yourself with the APA format and practice using it correctly. It is used for most writing assignments for your degree. Visit the Writing Center in the Student Success Center, under the Resources tab in Loud-cloud for APA paper templates, citation examples, tips, etc. Points will be deducted for poor use of APA format or absence of APA format (if required). Cite all sources of information! When in doubt, cite the source. Paraphrasing also requires a citation. I highly recommend using the APA Publication Manual, 6th edition.

  • Use of Direct Quotes

I discourage over-utilization of direct quotes in DQs and assignments at the Master’s level and deduct points accordingly. As Masters’ level students, it is important that you be able to critically analyze and interpret information from journal articles and other resources. Simply restating someone else’s words does not demonstrate an understanding of the content or critical analysis of the content. It is best to paraphrase content and cite your source.

  • LopesWrite Policy

For assignments that need to be submitted to Lopes Write, please be sure you have received your report and Similarity Index (SI) percentage BEFORE you do a “final submit” to me. Once you have received your report, please review it. This report will show you grammatical, punctuation, and spelling errors that can easily be fixed. Take the extra few minutes to review instead of getting counted off for these mistakes. Review your similarities. Did you forget to cite something? Did you not paraphrase well enough? Is your paper made up of someone else’s thoughts more than your own? Visit the Writing Center in the Student Success Center, under the Resources tab in Loud-cloud for tips on improving your paper and SI score.

  • Late Policy

The university’s policy on late assignments is a 10% penalty PER DAY LATE. This also applies to late DQ replies. Please communicate with me if you anticipate having to submit an assignment late. I am happy to be flexible, with advance notice. We may be able to work out an extension based on extenuating circumstances. If you do not communicate with me before submitting an assignment late, the GCU late policy will be in effect. I do not accept assignments that are two or more weeks late unless we have worked out an extension. As per policy, no assignments are accepted after the last day of class. Any assignment submitted after midnight on the last day of class will not be accepted for grading.

  • Communication

Communication is so very important. There are multiple ways to communicate with me: Questions to Instructor Forum: This is a great place to ask course content or assignment questions. If you have a question, there is a good chance one of your peers does as well. This is a public forum for the class. Individual Forum: This is a private forum to ask me questions or send me messages. This will be checked at least once every 24 hours.
