Preparation Health care leaders often find that resolving a problem in delivering safe, high-quality health care requires the combined effort of a number of agencies, departments, and individuals. For this assessment, identify a quality-related situation or problem relevant to your professional practice or area of interest that requires the collaborative effort of several departments and individuals to resolve. You will assume the role of team leader and draft a proposal for the organization’s executive leadership that outlines why this interprofessional team is necessary, who should take part in the collaboration, how the team will function, and how progress will be reported. A project proposal should identify the situation or problem, identify the causes or contributing factors, present a plan of action, provide details about the plan, and provide reasons why the plan should be undertaken. Note: Remember, you can submit all or a portion of your draft to Smarthinking for feedback before you submit the final version of your proposal for this assessment. However, be mindful of the turnaround time for receiving feedback if you plan on using this free service.

Proposal Requirements Write a proposal for assembling an interprofessional team. As you begin drafting your proposal, consider your goal and the needs of the leaders whose decisions will be based on the information you provide. Note: The requirements outlined below correspond to the grading criteria in the Interprofessional Collaboration scoring guide. At a minimum, be sure to address each point. In addition, you are encouraged to review the performance level descriptions for each criterion to see how your work will be assessed. You are provided an example proposal that illustrates what proficient-level work for this assessment looks like. Proposal Format and APA Style Use the simplified Project Proposal Template, linked in the Resources, to draft your proposal. You may organize the content of your proposal in a format used in your organization. An abstract is not required. Your proposal should be 4–6 double-spaced pages in length, not including the title page and reference page. Apply correct APA formatting to all in-text citations and references. Use Times New Roman, 12-point font.

Writing Be concise, but thorough. Your readers need sufficient and accurate information on which to base their decisions, and do not have time to sift through information that is not pertinent. Express your main points, arguments, and conclusions coherently. Use correct grammar and mechanics. Be sure to support your claims, arguments, and conclusions with credible evidence from 3–5 current, scholarly or professional sources. Proofread your writing.

Plan Content Analyze the factors that have contributed to the problem. ( Hint: Examine the reasons for ineffective communication, not just the communication itself. Reasons may include staff burnout, workload, staffing shortfalls, attitudes, assumptions, and so on.) Consider whether certain combinations of factors might be contributing to the problem. Explain why an interprofessional team is needed to address the problem and achieve expected project outcomes. Determine who should be on the team. Identify the disciplines or areas team members should come from and any specific job titles prospective team members should hold. Describe the knowledge and expertise each discipline has to offer in resolving the situation. In addition, consider the level of creativity and problem-solving skills that are needed. Assess the effectiveness of a particular leadership approach in building and maintaining interprofessional collaborative relationships.

Consider how effective that approach is in: Encouraging participation by all team members. Engaging reluctant or resistant team members. Maintaining a respectful platform for members to voice their ideas. Develop a strategy for communicating progress by the team to executive leadership. Determine which approach to communicating with leaders will be most effective and efficient. Determine whether you will have regular meetings and generate status reports. Develop a collaborative plan for resolving the problem. Identify the ethical or political issues that are relevant factors in your proposed solution. Identify the social, cultural, or economic factors that are relevant to your proposed solution. Explain how you would address the ethical, political, social, cultural, or economic factors you have identified as relevant to your plan. Describe your role as a leader in implementing the proposed solution. Determine how the practitioner-scholarmodel might be applied in resolving the problem. Consider how theory, research, and the published work in the field, in conjunction with your own experience and professional knowledge, might be used to develop strategies to analyze and resolve the problem. Be sure you read all of the requirements for this assessment and review the suggested resources to see if they may be useful in completing the assessment. This assessment provides an opportunity to determine how you can help advance your professional field by developing a strategy for managing diversity that recognizes the synergy achieved by having a diverse and inclusive workforce. By successfully completing this assessment, you will demonstrate your proficiency in the following course competencies and assessment criteria: Competency 1: Explain the concepts, principles, and characteristics of effective health care leadership. Determine the influence of effective leadership on the development of a diverse and culturally competent workforce.

Competency 3: Analyze standards of professional ethics and the principles of diversity and inclusion as applied by health care leaders in real-world situations. Outline a plan to recruit, hire, and retain a diverse workforce. Outline a plan for providing employee training on cultural awareness and sensitivity. Competency 4: Determine the influence of the practitioner-scholar role on professional practice and leadership development. Explain how the academic and research skills one develops as a practitioner-scholar can be of service in one’s role as a health care leader. Competency 5: Communicate effectively with diverse audiences, in an appropriate form and style, consistent with applicable organizational, professional, and scholarly standards. Write coherently to support a central idea, using correct grammar, mechanics, and APA formatting.

Preparation Use the following case study as the basis of this assessment. Imagine you are the manager of a new health care facility satellite office or community outreach center located in a diverse neighborhood. The parent organization opened the facility to address the underserved health care needs of the culturally diverse residents who were not using the services offered at the main facility. The first month of operation saw a very high volume of patients, but since then, the numbers have dropped off drastically. Executive leadership wants to understand why this has happened and what you plan to do about it. After careful examination of all aspects of the facility, and talking with some of the neighborhood residents, you have determined that the residents do not feel comfortable coming to the facility because the staff does not represent the diversity of the neighborhood. Staff members were hired for their skill, but their diversity was not considered. Now, you must report to executive leadership what you have learned and how you intend to manage diversity. Note: Remember, you can submit all or a portion of your draft to Smarthinking for feedback before you submit the final version of your proposal for this assessment. Report Content Outline a plan to recruit, hire, and retain a more diverse workforce. Note: Your outline should be a high-level overview of a plan, not a detailed plan. Other courses in your program will go into human resource practices and diversity in depth. In drafting your plan, consider the following questions: How does a leader develop a diverse workforce? Where will you look for more diverse employees? Will you post job ads? What will the interviewing and hiring process look like? Who will do the interviewing? Who will make the final decisions? What kind of incentives will you offer diverse employees to retain them? What do you need to know about the culture and values of diverse employees before developing a retention plan? How will you address issues related to diversity that result in conflict?

Outline a plan for the ongoing training and professional development of existing staff in cultural awareness and sensitivity. Consider how your plan can: Improve communications and interpersonal skills. Help overcome bias and resistance to change. Report Content

Address the following points in a 4–6 page report: Evaluate the expected outcomes of the population health improvement initiative that were, and were not, achieved. Describe the outcomes that were achieved, their positive effects on the community’s health, and any variance across demographic groups. Describe the outcomes that were not achieved, the extent to which they fell short of expectations, and any variance across demographic groups. Identify the factors (for example: institutional, community, environmental, resources, communication) that may have contributed to any achievement shortfalls. Propose a strategy for improving the outcomes of the population health improvement initiative, or ensuring that all outcomes are being addressed, based on the best available evidence. Describe the corrective measures you would take to address the factors that may have contributed to achievement shortfalls. Cite the evidence (from similar projects, research, or professional organization resources) that supports the corrective measures you are proposing. Explain how the evidence illustrates the likelihood of improved outcomes if your proposed strategy is enacted.

Develop an approach to personalizing patient care that incorporates lessons learned from the population health improvement initiative outcomes. Explain how the outcomes and lessons learned informed the decisions you made in your approach for personalizing care for the patient with a health condition related to the population health concern addressed in the improvement initiative. Ensure that your approach to personalizing care for the individual patient addresses the patient’s: Individual health needs. Economic and environmental realities. Culture and family.

Explain the benefits, to both the organization and the community it serves, of having a diverse and culturally competent workforce. Explain the relationship between health care organizations and communities. Explain the role that diversity plays in providing culturally competent care and access for the under-served in a community. Determine the influence of effective leadership on the development of a diverse and culturally competent workforce. Explain the importance of cultural competence for leaders in an organization. Explain how the academic and research skills you develop as a practitioner-scholar can serve you in your role as an effective health care leader. Preparation

Health care leaders often find that resolving a problem in delivering safe, high-quality health care requires the combined effort of a number of agencies, departments, and individuals. Capella FP5004 Assessment 1

Proposal Requirements

Write a proposal for assembling an interprofessional team.

Writing

Be concise, but thorough.

Note: Your instructor may also use the Writing Feedback Tool to provide feedback on your writing. In the tool, click the linked resources for helpful writing information.

Preparation You have been presented with a number of patient case files in the Evidence-Based Patient-Centered Care media piece. You reviewed each case, selected one case for further research, and created draft evidence-based concept map to illustrate an approach to individualized care for the patient. In this assessment, you will build upon and refine your draft concept map and develop a supporting narrative. Create your concept map and narrative as separate documents. Be sure to note the areas where you need to include your evidence-based support and where you need to make clear your strategies for communicating information to the patient and the patient’s family. Note: Many organizations use the spider style of concept maps (see the Taylor & Littleton-Kearney article for an example). Also, if a specific style of concept map is used in your current care setting, you may use it in this assessment. Create your concept map and narrative as separate documents. Be sure to note the areas where you need to include your evidence-based support and where you need to make clear your strategies for communicating information to the patient and the patient’s family.

Apply correct APA formatting to all in-text citations and references. Attach a reference list to your narrative. Concept Map Develop a concept map for the individual patient, based upon the best available evidence for treating your patient’s health, economic, and cultural needs. Narrative Develop a narrative (2–4 pages) for your concept map. Analyze the needs of your patient and their family, and determine how those needs will influence a patient-centered concept map. Consider how your patient’s economic situation and relevant environmental factors may have contributed to your patient’s current condition or affect their future health. Consider how your patient’s culture or family should influence your concept map. Justify the value and relevance of the evidence you used as the basis of your concept map. Explain why your evidence is valuable and relevant to your patient’s case. Explain why each piece of evidence is appropriate for both the health issue you are trying to correct and for the unique situation of your patient and their family. Propose relevant and measurable criteria for evaluating the degree to which the desired outcomes of your concept map were achieved. Explain why your proposed criteria are appropriate and useful measures of success. Explain how you will communicate specific aspects of the concept map to your patient and their family in an ethical, culturally sensitive, and inclusive way. Ensure that your strategies: Promote honest communications. Facilitate sharing only the information you are required and permitted to share. Are mindful of your patient’s culture. Enable you to make complex medical terms and concepts understandable to your patient and their family, regardless of language, disabilities, or level of education.

We will process your orders through multiple stages and checks to ensure that what we are delivering to you, in the end, is something that is precise as you envisioned it. All of our essay writing service products are 100% original, ensuring that there is no plagiarism in them. The sources are well-researched and cited so it is interesting. Our goal is to help as many students as possible with their assignments, i.e. our prices are affordable and services premium.

  • Discussion Questions (DQ)

Initial responses to the DQ should address all components of the questions asked, including a minimum of one scholarly source, and be at least 250 words. Successful responses are substantive (i.e., add something new to the discussion, engage others in the discussion, well-developed idea) and include at least one scholarly source. One or two-sentence responses, simple statements of agreement or “good post,” and responses that are off-topic will not count as substantive. Substantive responses should be at least 150 words. I encourage you to incorporate the readings from the week (as applicable) into your responses.

  • Weekly Participation

Your initial responses to the mandatory DQ do not count toward participation and are graded separately. In addition to the DQ responses, you must post at least one reply to peers (or me) on three separate days, for a total of three replies. Participation posts do not require a scholarly source/citation (unless you cite someone else’s work). Part of your weekly participation includes viewing the weekly announcement and attesting to watching it in the comments. These announcements are made to ensure you understand everything that is due during the week.

  • APA Format and Writing Quality

Familiarize yourself with the APA format and practice using it correctly. It is used for most writing assignments for your degree. Visit the Writing Center in the Student Success Center, under the Resources tab in Loud-cloud for APA paper templates, citation examples, tips, etc. Points will be deducted for poor use of APA format or absence of APA format (if required). Cite all sources of information! When in doubt, cite the source. Paraphrasing also requires a citation. I highly recommend using the APA Publication Manual, 6th edition.

  • Use of Direct Quotes

I discourage over-utilization of direct quotes in DQs and assignments at the Master’s level and deduct points accordingly. As Masters’ level students, it is important that you be able to critically analyze and interpret information from journal articles and other resources. Simply restating someone else’s words does not demonstrate an understanding of the content or critical analysis of the content. It is best to paraphrase content and cite your source.

  • LopesWrite Policy

For assignments that need to be submitted to Lopes Write, please be sure you have received your report and Similarity Index (SI) percentage BEFORE you do a “final submit” to me. Once you have received your report, please review it. This report will show you grammatical, punctuation, and spelling errors that can easily be fixed. Take the extra few minutes to review instead of getting counted off for these mistakes. Review your similarities. Did you forget to cite something? Did you not paraphrase well enough? Is your paper made up of someone else’s thoughts more than your own? Visit the Writing Center in the Student Success Center, under the Resources tab in Loud-cloud for tips on improving your paper and SI score.

  • Late Policy

The university’s policy on late assignments is a 10% penalty PER DAY LATE. This also applies to late DQ replies. Please communicate with me if you anticipate having to submit an assignment late. I am happy to be flexible, with advance notice. We may be able to work out an extension based on extenuating circumstances. If you do not communicate with me before submitting an assignment late, the GCU late policy will be in effect. I do not accept assignments that are two or more weeks late unless we have worked out an extension. As per policy, no assignments are accepted after the last day of class. Any assignment submitted after midnight on the last day of class will not be accepted for grading.

  • Communication

Communication is so very important. There are multiple ways to communicate with me: Questions to Instructor Forum: This is a great place to ask course content or assignment questions. If you have a question, there is a good chance one of your peers does as well. This is a public forum for the class. Individual Forum: This is a private forum to ask me questions or send me messages. This will be checked at least once every 24 hours. FP6011 Full Course

Capella FPX4010 Assessment 1

For this assessment you will create a 5-10 minute video reflection on an experience in which you collaborated interprofessionally, as well as a brief discussion of an interprofessional collaboration scenario and how it could have been better approached.

Interprofessional collaboration is a critical aspect of a nurse’s work. Through interprofessional collaboration, practitioners and patients share information and consider each other’s perspectives to better understand and address the many factors that contribute to health and well-being (Sullivan et al., 2015). Essentially, by collaborating, health care practitioners and patients can have better health outcomes. Nurses, who are often at the frontlines of interacting with various groups and records, are full partners in this approach to health care.

Reflection is a key part of building interprofessional competence, as it allows you to look critically at experiences and actions through specific lenses. From the standpoint of interprofessional collaboration, reflection can help you consider potential reasons for and causes of people’s actions and behaviors (Saunders et al., 2016). It also can provide opportunities to examine the roles team members adopted in a given situation as well as how the team could have worked more effectively.

Professional Context

This assessment will help you to become a reflective practitioner. By considering your own successes and shortcomings in interprofessional collaboration, you will increase awareness of your problem-solving abilities. You will create a video of your reflections, including a discussion of best practices of interprofessional collaboration and leadership strategies, cited in the literature.

Scenario

As part of an initiative to build effective collaboration at your Vila Health site, where you are a nurse, you have been asked to reflect on a project or experience in which you collaborated interprofessionally and examine what happened during the collaboration, identifying positive aspects and areas for improvement.

You have also been asked to review a series of events that took place at another Vila Health location and research interprofessional collaboration best practices and use the lessons learned from your experiences to make recommendations for improving interprofessional collaboration among their team. Your task is to create a 5–10 minute video reflection with suggestions for the Vila Health team that can be shared with leadership as well as Vila Health colleagues at your site. Note: If you require the use of assistive technology or alternative communication methods to participate in this activity, please contact [email protected].edu to request accommodations. If, for some reason, you are unable to record a video, please contact your faculty member as soon as possible to explore options for completing the assessment.

Instructions

Using Kaltura, record a 5–10 minute video reflection on an interprofessional collaboration experience from your personal practice, proposing suggestions on how to improve the collaboration presented in the Vila Health: Collaboration for Change activity.

Be sure that your assessment addresses the following criteria. Please study the scoring guide carefully so you will know what is needed for a distinguished score:

Reflect on an interdisciplinary collaboration experience, noting ways in which it was successful and unsuccessful in achieving desired outcomes.

Identify how poor collaboration can result in inefficient management of human and financial resources, citing supporting evidence from the literature.

Identify best-practice leadership strategies from the literature that would improve an interdisciplinary team’s ability to achieve its goals, citing at least one author from the literature.

Identify best-practice interdisciplinary collaboration strategies to help a team achieve its goals and work together, citing the work of at least one author.

Communicate in a professional manner, is easily audible, and uses proper grammar. Format reference list in current APA style.

You will need to relate an experience that you have had collaborating on a project. This could be at your current or former place of practice, or another relevant project that will enable you to address the requirements. In addition to describing your experience, you should explain aspects of the collaboration that helped the team make progress toward relevant goals or outcomes, as well as aspects of the collaboration that could have been improved.

A simplified gap-analysis approach may be useful:

What happened?
What went well?
What did not go well?
What should have happened?

After your personal reflection, examine the scenario in the Vila Health activity and discuss the ways in which the interdisciplinary team did not collaborate effectively and the negative implications for the human and financial resources of the interdisciplinary team and the organization as a whole.

Building on this investigation, identify at least one leadership best practice or strategy that you believe would improve the team’s ability to achieve their goals. Be sure to identify the strategy and its source or author and provide a brief rationale for your choice of strategy.

Additionally, identify at least one interdisciplinary collaboration best practice or strategy to help the team achieve its goals and work more effectively together. Again, identify the strategy, its source, and reasons why you think it will be effective.

You are encouraged to integrate lessons learned from your self-reflection to support and enrich your discussion of the Vila Health activity.

You are required to submit an APA-formatted reference list for any sources that you cited specifically in your video or used to inform your presentation. The Example Kaltura Reflection will show you how to cite scholarly sources in the context of an oral presentation.

Refer to the Campus tutorial Using Kaltura [PDF] as needed to record and upload your reflection.

Additional Requirements

References: Cite at least 3 professional or scholarly sources of evidence to support the assertions you make in your video. Include additional properly cited references as necessary to support your statements.
APA Reference Page: Submit a correctly formatted APA reference page that shows all the sources you used to create and deliver your video.
You may wish to refer to the Campus APA Module for more information on applying APA style. FPX4010 Assessment 1
